Health Canada Approves Tevimbra for Rare Throat Cancer, Marking First Immunotherapy Option for Nasopharyngeal Carcinoma

Breakthrough Results from Phase 3 Trial
The Health Canada approval was supported by results from the RATIONALE-309 study, a multicentre, Phase 3 randomized, double-blind trial. The study met its primary endpoint at the first prespecified interim analysis, demonstrating that Tevimbra significantly prolonged progression-free survival in the intent-to-treat population with a hazard ratio of 0.52 (95% CI: 0.38, 0.73; p<0.0001).
The combination therapy showed a 48% reduction in the risk of disease progression or death compared to placebo plus chemotherapy. Median progression-free survival in the Tevimbra plus chemotherapy arm reached 9.2 months compared to 7.4 months in the placebo plus chemotherapy arm.
Safety Profile and Tolerability
Tevimbra plus chemotherapy was generally well tolerated in the clinical trial. The most common grade 3 or higher treatment-emergent adverse events (≥20%) associated with the combination therapy were neutropenia, anemia, and leukopenia.
Addressing a Rare but Significant Disease
Nasopharyngeal carcinoma (搜索) is a rare form of head and neck cancer (搜索) that originates in the upper part of the throat behind the nose. The disease affects less than one in 100,000 Canadians in the general population, but shows significantly higher incidence rates in certain communities, particularly Inuit populations in Nunavut and the Northwest Territories, where rates reach approximately 10 per 100,000.

Clinical Impact and Accessibility
The approval represents an important milestone for Canadian patients, as accessing immunotherapy for first-line treatment of recurrent or metastatic NPC has been challenging. NPC is often diagnosed at advanced stages due to its deep anatomical location and mild early symptoms, making early detection difficult.
By the end of 2025, an estimated 8,100 Canadians will be diagnosed with head and neck cancer (搜索), with 2,200 deaths expected in 2025. While NPC statistics are not reported separately in Canada, they are captured within the broader head and neck cancer category.
Drug Mechanism and Development
Tislelizumab is a humanized immunoglobulin G4 (IgG4) variant monoclonal antibody against PD-1 (搜索). The drug works by binding to the extracellular domain of the PD-1 receptor, blocking interaction with both PD-L1 (搜索) and PD-L2 (搜索) ligands, thereby inhibiting T-cell proliferation and cytokine production.
The drug was first approved in China in 2019, followed by approvals in the European Union in 2023, and in the United States and Australia in 2024. Tevimbra is anticipated to be commercially available in Canada in the first half of 2026.
