Health system fragmentation across multiple hospitals and clinics is creating significant barriers to timely biomarker testing for lung cancer (搜索) patients, potentially delaying access to life-saving targeted therapies, according to Julia Rotow, MD, clinical director of the Lowe Center for Thoracic Oncology (搜索) at Dana-Farber Cancer Institute.
Speaking at the Institute for Value-Based Medicine (搜索) regional event in Boston, Rotow explained that understanding a patient's biomarker profile is "critical to offering effective first-line therapy" for non-small cell lung cancer (搜索) (NSCLC). However, the complex structure of American healthcare creates substantial obstacles to obtaining these results quickly.
Complex Healthcare Systems Delay Critical Testing
"Health care in this country is very complex. Patients' care may be spread across multiple different health care systems, multiple hospitals, and different clinics," Rotow noted. "That means their treatment in oncology may be at a different place from where they actually got their original diagnosis."
The fragmentation creates what Rotow describes as "mundane but real barriers," including simply getting physical access to tumor samples. Tissue must often be transferred from the hospital where the biopsy was performed to the clinic where testing will be conducted or to central testing companies, creating logistical challenges that can significantly delay results.
The timing issue is particularly acute because biomarker testing typically takes 2-3 weeks to return results. Patients often spend weeks or months in evaluation before receiving their diagnosis and seeing an oncologist, who is usually the first to order biomarker testing. "It's really hard with a serious illness like lung cancer (搜索), where we want to start therapy as often, and as quickly, as possible," Rotow emphasized.
Hospitalized Patients Face Additional Hurdles
The sickest patients at diagnosis face even greater challenges. Those hospitalized for symptoms or expedited evaluation encounter barriers to ordering complex genomic testing methods like next-generation sequencing, which are typically difficult to initiate in inpatient settings and are usually ordered by outpatient teams.
Compounding these issues are regulatory restrictions, including 14-day rules that limit therapy initiation. "Our patients can't wait 14 days to even start a 3-week timeline on a test," Rotow explained, describing these restrictions as "a real barrier to effective initiation of therapy."
Evolving Treatment Landscape Complicates Testing
The challenge extends beyond logistics to the rapidly evolving nature of lung cancer (搜索) treatment. The definition of comprehensive biomarker profiling continues to change as new actionable targets emerge. "What was comprehensive, say, 2 or 3 years ago might not be comprehensive today," Rotow observed.
This evolution requires oncologists to actively review patient biomarker profiles at each line of therapy, checking whether new actionable biomarkers have become available since initial testing. For thoracic oncologists, this represents "a very active and ongoing effort, longitudinally, over a patient's care."
Community Practices Face Amplified Challenges
The burden is particularly heavy for community oncologists, who must stay current across biomarkers and therapies for all cancer types, not just lung cancer (搜索). "That challenge is amplified in a community setting, a more general oncology setting," Rotow noted, emphasizing the critical need for ongoing education and continuing medical education access.
Patient Engagement as Solution
Rotow advocates for increased patient engagement as part of the solution. She encourages patients to ask their oncologists about new biomarkers and tests when considering therapy changes. "It's a great question for a patient to ask their oncologist as they're thinking about changing therapy: is there anything new? Is there any new biomarker? Is there a new test that we should be sending?"
Such questions can prompt oncologists to review testing comprehensively, helping ensure patients don't miss opportunities for newer targeted therapies. "I really want to encourage the empowerment of patients to ask questions and be an active participant in care as well, because it really can help make a difference," Rotow concluded.
Without timely biomarker testing, patients may receive chemotherapy instead of targeted therapies, effectively denying them "all the advances of the last 15 years in the field," according to Rotow. The stakes make addressing these systemic barriers increasingly urgent as precision medicine continues to transform lung cancer (搜索) treatment.
