HelioLiver Blood Test Shows Superior Sensitivity Over Ultrasound for Early Hepatocellular Carcinoma Detection in Landmark CLiMB Study

The landmark trial showed HelioLiver achieved 47.8% sensitivity for overall HCC detection compared to 28.3% for ultrasound, with 60% of detected HCC cases at stage I.
Superior Performance for Early-Stage Disease
The blood-based test demonstrated particularly striking advantages for early-stage and small lesions. For tumors ≤4 cm, HelioLiver (搜索) achieved 37.8% sensitivity versus 13.5% for ultrasound. The performance gap was even more pronounced for lesions smaller than 2 cm, where HelioLiver detected 28.6% of cases while ultrasound detected none (0%). For T1/stage 1 HCC specifically, the test achieved 40.0% sensitivity compared to ultrasound's 10.0%.
"The CLiMB study underscores the limitations of current ultrasound-based surveillance and the promise of this multi-analyte test in detecting HCC at earlier, more treatable stages in patients with cirrhosis (搜索)," said Prof. Mindie H. Nguyen, Professor of Medicine at Stanford University School of Medicine and Lead Principal Investigator for the CLiMB trial.
HelioLiver (搜索) maintained statistical non-inferior specificity at 87.6% compared to ultrasound's 93.9% and showed higher sensitivity than the combination of ultrasound plus AFP testing (47.8% vs. 34.8%), with comparable negative predictive value (97.8% vs. 97.4%).
Addressing Critical Surveillance Gaps
The study results highlight significant gaps in current liver cancer (搜索) surveillance. According to the research, liver cancer ranks among the fastest-rising causes of cancer-related deaths, with incidence rates tripling and mortality doubling since 1980. When identified early, the five-year survival rate reaches 70%, yet only approximately 9% of cirrhosis (搜索) patients receive recommended semiannual ultrasound screening, with pooled adherence rates as low as 24% overall.
"Reliable early detection for HCC is lacking and the CLiMB trial results demonstrate that Helio's test has tremendous potential for addressing a large, unmet medical need," said Prof. Richard Van Etten, Director of the Chao Family Comprehensive Cancer Center at UC Irvine and Chief Medical Advisor at Helio Genomics (搜索). "Current imaging surveillance catches less than one-third of HCC cases and misses nearly all small, early-stage tumors."
Multi-Analyte Technology Platform
HelioLiver (搜索) combines cell-free DNA (搜索) methylation patterns, protein biomarkers (搜索), and patient demographics using a proprietary AI algorithm. The test detects tumor-associated epigenetic signals in circulating cell-free DNA alongside protein biomarkers, enabling earlier identification of cancer-associated signals, including cases where conventional imaging may be inconclusive.
The CLiMB trial included patients with cirrhosis (搜索) from diverse liver disease etiologies, with over half having either obesity and/or metabolic dysfunction-associated steatotic liver disease (搜索)/metabolic dysfunction steatohepatitis (搜索) (MASLD/MASH) - the group now driving the sharp rise in HCC cases.
Clinical Impact and Availability
The improved diagnostic capabilities may have significant implications for treatment outcomes. According to SEER database statistics, five-year survival rates for localized liver cancer (搜索) are approximately 37%, declining sharply to 13% and 3% for regional and distant disease, respectively. Earlier detection could increase the proportion of patients diagnosed at a resectable stage, potentially expanding eligibility for curative interventions such as minimally invasive ablation, tumor resection, or transplantation.
HelioLiver (搜索) is designed to fit into routine liver cancer (搜索) surveillance schedules with a simple blood draw, addressing barriers including scheduling difficulties, transportation challenges, and lack of access that contribute to low adherence rates for ultrasound screening. The test is now commercially available nationwide from Helio Genomics (搜索) through a collaboration with Quest Diagnostics, enabling physicians to order the test through existing clinical infrastructure.
