HHS Signals Major Changes Ahead for Childhood Vaccine Recommendations

The Department of Health and Human Services (搜索) has closed a comment period on a Federal Register request for information asking whether the existing types of federal vaccine recommendations should remain in place. Vaccines are currently recommended on a universal basis, through risk-based assessment for people with specific risk factors, or through what HHS calls shared clinical decision-making, described by the department as an individualized process between a patient and physician. The request also sought comment on "individual autonomy and religious freedom" and on communication practices needed to earn and maintain public trust in the federal childhood vaccine schedule (搜索).
In public comments, pediatric health advocacy groups, health attorneys and public health researchers argued that further changes would jeopardize public health and child safety. A group including the American Public Health Association (搜索), the Robert Wood Johnson Foundation (搜索) and Bruce G. Gellin, former deputy assistant secretary of Health and director of the HHS National Vaccine Program Office, wrote that "there simply is no basis for the RFI's implicit suggestion that the current recommendation framework produces unsafe results and needs fundamental revision." The group warned that categories proposed without context, evidence or a clear communication strategy would produce confusion and uncertainty, with the likely result that doses are missed and vaccine-preventable diseases recur. HHS has floated potential categories including vaccines recommended but not for babies, or recommended with other undefined caveats.
The review stems from an August 2026 executive order by President Donald Trump requiring HHS to revive a defunct childhood vaccine task force and reevaluate childhood vaccines. The United States is in its worst year for measles (搜索) since 1991, with 792 cases in Pennsylvania this year and four deaths in the state since January; two of those Pennsylvania deaths were removed from the CDC measles dashboard, which as of Sept. 18 listed no measles deaths. A comment from Georgetown University's McCourt School of Public Policy noted that patients overwhelmingly trust their personal healthcare providers, and a July KFF survey found 86% of respondents trust their own doctor, compared with 51% who trust the CDC and 34% who trust HHS Secretary Robert F. Kennedy Jr.
