High-Dose Flu Vaccine Linked to 55% Lower Alzheimer's Risk in Adults Over 65

A large retrospective study has revealed that high-dose influenza (搜索) vaccines are associated with a significantly lower risk of Alzheimer's disease (搜索) in adults aged 65 and older compared to standard-dose formulations. The research, published in Neurology and led by investigators at McGovern Medical School at UTHealth (搜索) in Houston, analyzed health data from approximately 165,000 older adults and found the high-dose vaccine reduced Alzheimer's risk by nearly 55% over roughly two years.
The study examined data from the IQVIA PharMetrics Plus for Academics database spanning August 2014 to July 2019, comparing 120,775 individuals who received high-dose vaccines with 44,022 who received standard-dose vaccines. Participants had a mean age of 73-74 years, with 56-57% being women, and were followed for an average of 14-19 months depending on the analysis method.
Enhanced Protection Through Higher Antigen Content
High-dose influenza (搜索) vaccines contain four times the antigen—the component that generates an immune response—of standard-dose vaccines. The Centers for Disease Control and Prevention (搜索) already recommends high-dose vaccines for all adults 65 and older because the immune system naturally weakens with age, making older adults less responsive to standard vaccines.
"I was stunned that, as a physician, I didn't know a higher dose was offered," said lead study author Paul Schulz, professor of neurology at McGovern Medical School at UTHealth (搜索), in a press release. Schulz previously led research linking general flu vaccination to a 40% reduction in Alzheimer's risk over four years.
Sex-Specific Benefits Observed
The protective effects showed notable differences between men and women. Sex-stratified analyses revealed stronger and earlier benefits in women, with significantly reduced risk observed in the first 13 months in the per-protocol analysis and through 17 months in the intention-to-treat analysis. The number needed to treat (NNT) for women was 417 in the per-protocol analysis.
In men, significant reductions emerged later, between 17 and 24 months in the intention-to-treat analysis with an NNT of 233, while per-protocol results in men were not statistically significant. Both groups saw significant benefits overall, but the effect appeared to last longer and be more consistent in women.
Sustained Benefits with Annual Vaccination
The research found that sustained annual high-dose vaccination over three years was associated with further reductions in Alzheimer's risk, with effects observed up to 27 months and an NNT of 294. In the per-protocol analysis, the maximum risk difference reached 0.0054 at 25 months, corresponding to an NNT of 185. The intention-to-treat analysis showed effects persisting up to 28 months, with a maximum risk difference of 0.0037 and an NNT of 270.
Potential Mechanisms Behind Protection
Researchers hypothesize that the protection may stem from enhanced immune responses that reduce inflammation. "Enhanced [flu vaccines] confer greater protection against influenza (搜索) infection, thereby decreasing risk of severe illness and the associated systemic inflammation that can promote neuroinflammation and neurodegeneration," the authors write.
Dr. Marc Siegel, a Fox News senior medical contributor not involved in the research, explained that as the vaccine directly affects the immune system, this interaction could decrease inflammation in the body and "thereby indirectly decrease Alzheimer's risk." He noted that "flu shots and their components do not cross the blood-brain barrier, meaning they aren't directly affecting brain cells."
Eric Topol, MD, professor and executive vice president at Scripps Research (搜索), told CIDRAP news that if the findings are true, the vaccine is "likely working via stimulating the immune system, raising its functionality in older adults who are prone to immunosenescence." Immunosenescence refers to the gradual deterioration of the immune system that occurs with aging.
Study Limitations and Future Research Needs
The researchers acknowledge several limitations, including the retrospective design using claims data, potential healthy-user bias, and short follow-up periods. The study used International Classification of Diseases codes and pharmacy claims to identify Alzheimer's cases, which may have led to some misclassification. Additionally, the research lacked mortality and socioeconomic data as potential confounding factors.
"This is not a cause/effect study," Siegel emphasized. "We can't conclude that the flu shot itself causes the effect; it could be something about the people who decide to take this shot."
The findings align with emerging research linking certain vaccines to delayed or prevented neurodegenerative disease. Recent studies have also connected the shingles vaccine to slowed neurodegeneration and decreased dementia (搜索) risk, particularly in women.
The researchers call for further prospective studies with longer follow-up periods, diverse populations, and biomarker-confirmed outcomes to validate these findings. Future research should investigate whether the association is driven by reduced influenza (搜索) burden or broader immunologic mechanisms, including trained immunity and reduced neuroinflammation.
With Alzheimer's disease (搜索) affecting more than 6 million Americans, most of them age 65 or older, these findings provide additional support for current CDC recommendations while highlighting the potential cognitive benefits of enhanced vaccination strategies in older adults.
