Addressing a Critical Treatment Gap
Malaria (搜索) in pregnancy poses devastating consequences across sub-Saharan Africa, where the disease is responsible for 20% of all stillbirths and 11% of all newborn deaths. The condition also contributes to approximately 10,000 maternal deaths globally each year and can cause severe maternal anemia, miscarriage, preterm delivery, and low birthweight.
Despite these alarming statistics, treatment options for pregnant women remain severely limited, particularly during the first trimester. According to the World Health Organization, 12.4 million pregnant women in sub-Saharan Africa were exposed to malaria (搜索) in 2023, yet therapeutic choices have remained stagnant for decades.
"With the threat of emerging resistance to malaria (搜索) drugs, alternatives are needed, and I think we all agree that we cannot afford to wait for another quarter of century [for treatment for pregnant women]," said Dr. Myriam El Gaaloul, head of clinical and regulatory sciences at Medicines for Malaria Venture (搜索) and principal investigator of the SAFIRE consortium (搜索).
Trial Design and Objectives
The SAFIRE trial will recruit pregnant women in their first trimester who have contracted malaria (搜索) across three African countries: Mali, Kenya, and Burkina Faso. The study aims to compare the safety and efficacy of three artemisinin-based combination therapies (ACTs).
Participants will receive one of three treatments: pyronaridine-artesunate (搜索) (PA), dihydroartemisinin-piperaquine (DP), or artemether-lumefantrine (AL). While PA and DP are approved for the general population, they have not yet been validated for use in early pregnancy. AL currently stands as the only WHO-approved therapy for first-trimester malaria (搜索) treatment.
The trial protocol includes comprehensive follow-up care throughout pregnancy until delivery, with newborns monitored for up to six months after birth to assess long-term safety outcomes.
Overcoming Historical Exclusion
The enrollment represents a significant shift from decades of excluding pregnant women from clinical trials. Dr. Jacqueline Kitulu, incoming president of the World Medical Association (搜索), emphasized that such exclusion "generates several ethical harms," including the risk that "clinical decisions are built on non-generalisable data."
"Exclusion can cause harm when interventions are later rolled out without safety or dosage data for larger segments of the population who are really excluded or under-represented groups," Kitulu explained during a recent webinar on under-represented populations in clinical trials.
The WHO only updated its malaria (搜索) treatment guidelines to include pregnant women in their first trimester in 2022, despite recommending artemisinin-based combination therapy (搜索) as the standard of care for malaria in 1998 – a 24-year gap that highlights the historical neglect of this population.
Community Engagement and Cultural Considerations
Before patient recruitment began, the SAFIRE consortium (搜索) conducted extensive formative research to ensure culturally appropriate recruitment strategies and participant materials that reflected local community realities.
"The formative research we conducted as part of community engagement was key in understanding socio-cultural beliefs and barriers that could hinder the recruitment and retention of participants," said Dr. Innocent Valea, SAFIRE co-principal investigator from The Institut de Recherche en Sciences de la Santé Burkina Faso.
Broader Implications for Clinical Research
The trial's launch coincides with growing international momentum to include under-represented populations in clinical research. The WHO recently launched the Global Clinical Trial Forum to strengthen the clinical trial ecosystem, with specific emphasis on including previously excluded populations.
"To advance the malaria (搜索) elimination agenda in a meaningful and sustainable manner, properly tolerated and effective options must be made available to everyone at risk of malaria, including pregnant women," said El Gaaloul, who also heads the Malaria in Mothers and Babies strategy at Medicines for Malaria Venture (搜索).
Prof Kassoum Kayentao of The Université des Sciences, des Techniques et des Technologies de Bamako in Mali, SAFIRE's co-principal investigator, noted that the trial will "fill the ethical gap of excluding pregnant women from clinical trials" while providing "necessary information to increase therapeutic options in the guidelines for the management of malaria (搜索) in all stages of pregnancy."
The physiological changes during pregnancy make dedicated research essential, as blood volume increases by approximately 40%, significantly affecting how medicines are absorbed and processed. Despite this, around 70% of pregnant women take prescription medicines, with most being used off-label due to lack of pregnancy-specific data.
"Enrolling the first patient into the SAFIRE trial is a decisive step forward that will help pave the way towards a more just and equitable future in the fight against this disease," El Gaaloul concluded.
