Histotripsy Gains Traction as Noninvasive Ultrasound-Based Liver Tumor Ablation Expands Across U.S. Centers

Rising cancer incidence is fueling demand for less invasive treatment options, and histotripsy (搜索)—a noninvasive ultrasound-based ablation technique—is drawing growing attention as radiologists work to define its role alongside established modalities. The technology, recently cleared by the U.S. Food and Drug Administration (搜索) for select liver tumors (搜索), is now expanding to new U.S. centers, with the University of Arkansas for Medical Sciences (UAMS) becoming the first provider to offer the procedure in Arkansas.
Unlike thermal ablation approaches such as microwave, radiofrequency, and cryoablation, histotripsy (搜索) relies on mechanical tissue destruction. "Histotripsy utilizes high-amplitude, very short (microsecond) focused-ultrasound pulses to generate a cavitation bubble cloud whose rapid expansion and collapse mechanically destroys target tumor tissue," said Mina Makary, MD, an interventional radiologist in the Department of Radiology at The Ohio State University Wexner Medical Center and an early adopter of the technology. The rapid expansion and collapse of these microbubbles mechanically disrupt cells, leaving acellular debris that is typically reabsorbed over several months.
First described by investigators at the University of Michigan, Ann Arbor, in the early 2000s, histotripsy (搜索) derives its name from the Greek words histo (soft tissue) and tripsy (breakdown). Its mechanism has drawn interest for its potential to improve precision and limit injury to surrounding structures, and it is being explored for patients who are ineligible for surgical resection or other locoregional treatment options.
FDA Clearance and Clinical Evidence
In 2023, the FDA cleared the first histotripsy (搜索) system for the treatment of liver tumors (搜索) based on data from the multicenter HOPE4LIVER trial. "The FDA-reviewed clinical evidence was structured around safety and short-term ablation performance goals," Dr. Makary said. "Clinicians should interpret this clearance as regulatory confirmation of an FDA-cleared capability to noninvasively destroy liver tumor tissue with a short-term safety and technical-success profile that met predefined goals. Long-term efficacy and outcomes have not been evaluated."
While the early-phase THERESA trial and the HOPE4LIVER study demonstrated favorable safety and very short technical efficacy for tumors measuring approximately 3–4 cm, questions remain regarding scalability for multifocal or diffuse hepatic disease and long-term outcomes such as progression-free and overall survival.
Clinical Adoption at UAMS
At UAMS, Dr. James Meek, an interventional radiologist, described histotripsy (搜索) as "a non-invasive ultrasound-based method for destroying liver tumors (搜索)" that works by focusing "high intensity ultrasound waves to destroy the tumor tissue." The procedure can be used for both primary and metastatic liver tumors, including tumors that have spread to the liver from other parts of the body.
Patient selection remains critical. Candidates generally need tumors that are "small, four centimeters or less," and in locations that are visible on ultrasound. Dr. Meek noted that "as the technology matures, that may expand into other areas as well."
Treatment time typically runs "about a 20 to 30 minute treatment per tumor," depending on location, with a typical session taking "90 minutes to two hours in total." The procedure requires general anesthesia "to help keep the tumors still during the treatment." Histotripsy (搜索) is performed on an outpatient basis, and post-procedure pain "usually can be controlled with relatively mild pain medicine."
Early Outcomes and Current Limitations
Dr. Meek reported that early results have been strong for appropriately selected patients. "The success rate's been excellent for well-chosen patients," he said, noting that tumors that are difficult to target or too large are less likely to have good outcomes. In patients treated so far with well-visible tumors, "they've had a great outcome so far."
Histotripsy (搜索) remains "still kind of a niche option," Dr. Meek acknowledged, and is not the best choice for every patient. "Often this may be a part of a kind of multi-modality therapy," he said, adding that while it can be "a great option" for some patients, for others "it may not be," and "for certain patients this is not going to be the optimal therapy." UAMS is currently the only provider offering histotripsy in Arkansas, though the technology is available in some neighboring states.
