Dramatic Mortality Reductions Across Age Groups
The study, led by Queen Mary researchers Peter Sasieni and Milena Falcaro, analyzed cervical cancer (搜索) mortality data spanning 2001 to 2024 for women aged 20 to 34 in England. The results were striking: among women aged 20–24 between 2020 and 2024—approximately 80–90% of whom were vaccinated at ages 12–13 in 2008—zero cervical cancer deaths were recorded, compared to an expected 23.1 deaths based on historical rates.
For women offered the HPV vaccine through England's catch-up campaign targeting those up to age 18, the mortality reductions were similarly pronounced. Among women aged 20–24 between 2015 and 2019, where vaccination coverage ranged from 63% to 87%, cervical cancer (搜索) deaths fell by 80%. In the 25–29 age group during 2020–2024, mortality declined by 69%.
England's Vaccination Program: A Model for Elimination
England implemented its school-based national HPV vaccination program in 2008, initially targeting 12- and 13-year-old girls. Prior to the COVID-19 pandemic, coverage rates reached between 80% and 90% in this age group. A catch-up campaign extended vaccination to girls aged 14 to 18 over the subsequent two years. The program has contributed to a 90% drop in HPV infections among adolescents, positioning England closer to its goal of eliminating cervical cancer (搜索) as a public health problem by 2040.
As of February 2025, 148 countries associated with the World Health Organization have incorporated the HPV vaccine into their national immunization programs. HPV remains the most common sexually transmitted infection globally, encompassing more than 200 known types. While most infections clear spontaneously, certain high-risk strains cause nearly all cervical cancer (搜索) diagnoses.
Expert Commentary and Cautions
In an invited commentary also published in The Lancet, Dr. Allison Portnoy, assistant professor of global health at Boston University School of Public Health, described the study as "an important contribution to the literature on the population-level impact of HPV vaccination by connecting mortality data and HPV vaccine coverage data." However, she noted that these are "population-level rather than individual-level data," and cautioned that other factors may have contributed to the observed mortality decline.
"Improvements in and expansion of cervical cancer (搜索) screening, as well as increased HPV vaccination, may have been instrumental in the observed mortality decline," Dr. Portnoy wrote. She also highlighted the potential role of herd immunity, as vaccinated individuals are less likely to transmit HPV to unvaccinated individuals.
Dr. Portnoy further emphasized the need for continued surveillance: "Because vaccinated cohorts are still relatively young, continued and longer-term follow-up will be important to assess whether these reductions persist into older ages when cervical cancer (搜索) mortality risk is higher and when different HPV genotypes might contribute to the burden differentially by age."
Global Implications and Equity Considerations
The study underscores the critical importance of sustained investment in national vaccination programs and cancer screening infrastructure. Dr. Portnoy warned that without concerted effort, "inequities in access to the HPV vaccine will continue to widen, especially in low-income countries, where HPV rates are highest."
"The findings from England therefore provide an important benchmark for what could be achievable when high vaccination coverage is sustained over time," she wrote, adding that "realizing similar gains globally will require intentional implementation and delivery strategies to mitigate inequalities in cervical cancer (搜索) burden."
