A systematic review and meta-analysis examining 13 studies found that hydroxychloroquine significantly reduces proteinuria (搜索) levels in patients with primary glomerular diseases. The analysis, which included 1010 patients, demonstrated a substantial reduction in 24-hour urinary protein levels (MD, -0.69; 95% CI, -0.79 to -0.59; P <.00001) following hydroxychloroquine treatment.
The research addressed a critical knowledge gap, as most previous studies focused on hydroxychloroquine's role in secondary glomerular diseases such as lupus nephritis (搜索). "Most available studies have focused on hydroxychloroquine's role in secondary glomerular diseases, such as lupus nephritis, leaving its potential for primary glomerular diseases underexplored," noted Gerry Mathew, an associate professor of nephrology at SRM University in India, and colleagues.
Disease-Specific Efficacy Patterns
The meta-analysis revealed varying effectiveness across different primary glomerular disease (搜索) subtypes. Among 1161 patients with IgA nephropathy (搜索) and membranous nephropathy (搜索), subgroup analysis showed significant proteinuria (搜索) reduction in 797 patients with IgA nephropathy (MD, -0.67; 95% CI, -0.74 to -0.60; P <.00001). Notably, patients with membranous nephropathy experienced an even more substantial reduction (MD, -3.00; 95% CI, -4.46 to -1.53; P <.0001), though with high heterogeneity across studies.
Regarding kidney function preservation, the analysis found no significant overall improvement in estimated glomerular filtration rate (eGFR (搜索)) after hydroxychloroquine treatment. However, a beneficial effect was specifically observed in patients with IgA nephropathy (搜索) (MD, -2.65; 95% CI, -5.16 to -0.14).
Duration-Dependent Benefits
The research demonstrated that hydroxychloroquine's proteinuria (搜索)-reducing effects were consistent regardless of treatment duration. Subgroup analysis revealed significant proteinuria decrease in patients treated for both short-term (MD, -0.49; 95% CI, -0.66 to -0.32; P <.00001) and long-term (MD, -0.74; 95% CI, -0.81 to -0.67; P <.00001) durations compared to baseline levels.
Lupus Nephritis Protection
Complementary research examining 209 people with lupus nephritis (搜索) found that hydroxychloroquine use was associated with substantial kidney protection benefits. The study revealed that 33% and 23% of patients experienced eGFR (搜索) decline of ≥30% or ≥40%, respectively, along with reduced annual eGFR slope decline.
Most significantly, hydroxychloroquine use was linked to a 77% reduced risk of eGFR (搜索) decline by ≥30% in people with stage 3 or higher chronic kidney disease (搜索). The therapy decreased annual eGFR slope decline by 5.12 and 3.17 ml/min/1.73 m² in the first 5- and 10-years post-diagnosis, respectively.
Safety Profile and Clinical Implications
The safety analysis across the primary glomerular disease (搜索) studies documented 69 adverse events total. Gastrointestinal and mucocutaneous effects were the most commonly reported, each accounting for 20 cases and representing approximately 58% of all adverse events.
These findings have significant implications for clinical practice, particularly given that glomerular disease is a prominent cause of renal impairment, leading to 10-15% of end-stage renal disease cases in the United States. Current treatment relies primarily on immunosuppressive agents associated with serious long-term side effects and loss of efficacy over time.
"Addressing these gaps is critical for evaluating hydroxychloroquine as a viable therapeutic alternative for primary glomerular diseases and for reducing the dependency on traditional immunosuppressive therapies with high side-effect profiles," the researchers emphasized.
The research supports incorporating hydroxychloroquine as a crucial element of kidney disease management protocols, not just for systemic benefits, but also for its potential to preserve kidney function across multiple disease contexts.
