Hypofractionated Radiotherapy Shows Promise for Breast Cancer Treatment with Reduced Treatment Time

French Trial Validates 3-Week Radiotherapy Protocol
The UNICANCER HypoG-01 phase III trial, published in The Lancet, established that hypofractionated radiotherapy (搜索) delivered over 3 weeks is as effective as the standard 5-week protocol for early breast cancer (搜索) patients requiring nodal irradiation. The multicenter study randomized 1,221 women between September 2016 and March 2020 to receive either 40 Gy in 15 fractions over 3 weeks or the conventional 50 Gy in 25 fractions over 5 weeks.
After a median follow-up of 4.8 years, the primary endpoint of ipsilateral arm lymphedema (搜索) showed noninferiority for the shorter regimen. Lymphedema occurred in 143 patients (23.4% cumulative incidence at 3 years) in the 3-week group compared to 132 patients (22.2% cumulative incidence) in the 5-week group, with a hazard ratio of 1.02 (95% CI = 0.79–1.31, P < .001 for noninferiority).
"Three-week radiotherapy (搜索) (40 Gy in 15 fractions) was found to be noninferior to 5-week radiotherapy (50 Gy in 25 fractions) for arm lymphedema (搜索) risk and was comparably safe regarding other late normal tissue effects," concluded Dr. Sofia Rivera and colleagues from Institut Gustave Roussy.
Safety Profile Favors Shorter Treatment
The safety analysis revealed fewer grade 3 or higher adverse events in the 3-week group (8%) compared to the 5-week group (13%). Common adverse events across both groups included pain (69%), fatigue (65%), and fibrosis (47%). Serious adverse events were reported in 17 patients receiving 3-week treatment versus 15 patients in the 5-week group.
Ultra-Hypofractionation Shows Promise in Advanced Disease
A separate retrospective study presented at the 43rd Miami Breast Cancer (搜索) Conference explored even more aggressive fractionation schedules. Researchers evaluated 45 patients with locally advanced breast cancer (搜索) who received ultra-hypofractionated radiotherapy (搜索) consisting of 25 Gy delivered in just 5 fractions to the breast and regional lymph nodes.
The patient population was predominantly high-risk, with 88.9% having cT4 tumors and 80% having hormone receptor (搜索)-positive disease. Clinical response rates showed 46.6% of patients achieved favorable outcomes, while 4.4% had no response and 15.5% experienced progression. Notably, 71.1% of patients received neoadjuvant chemotherapy (搜索), and 88.9% were treated with curative intent.
Surgical outcomes were encouraging, with 92.9% of patients achieving pathological downstaging. Total mastectomy plus axillary lymph node dissection was performed in 42% of patients, with pathology reports available for 74%.
Clinical Implications and Future Directions
Carolina Gabler from the University of Diego Portales (搜索) noted that ultra-hypofractionation can be "useful in curative and palliative settings" and provides "promising options for extensive or initially unresectable locally advanced breast cancer (搜索)." However, she emphasized that "further prospective data are needed."
The technical considerations of hypofractionated radiotherapy (搜索) were highlighted by Dr. Sunil Dutta from Emory University, who explained that higher doses per fraction can increase the risk of hot spots and cosmetic complications, particularly in patients undergoing immediate reconstruction. "If you can do the 15 fractions and get good coverage of your targets and sparing of the organs at risk, I would say that's a great option," Dutta noted.
These studies collectively support the feasibility of shortened radiotherapy (搜索) courses in breast cancer (搜索) treatment, potentially improving patient convenience and healthcare resource utilization while maintaining therapeutic efficacy. The evidence is particularly strong for the 3-week hypofractionated approach in early breast cancer (搜索), while ultra-hypofractionation in advanced disease shows promise but requires further validation through prospective trials.
