A newly approved immunotherapy is reshaping outcomes for patients with small cell lung cancer (搜索) (SCLC), a notoriously aggressive malignancy that has long defied durable treatment responses. Imdelltra, a bispecific T-cell engager (BiTE) therapy, is being described as a "game changer" by clinicians at Oncology Hematology Care (搜索) (OHC) in Cincinnati, where at least two patients have experienced dramatic responses.
"This is a disease that comes on like a hurricane. A patient is well one minute and literally needing hospitalization the next," said Dr. David Waterhouse, chief innovation officer at OHC. "Small cell lung cancer (搜索), historically, has been a nearly untreatable disease. Lots of drugs worked, but none of them worked very long."
Mechanism and Clinical Experience
Imdelltra belongs to the BiTE therapy class, a form of immunotherapy that redirects a patient's own T-cells to bring them into direct contact with cancer cells, facilitating targeted tumor destruction. OHC participated in clinical trials for the drug and began offering it to patients shortly after the FDA approved it last year.
Patients receive Imdelltra treatments every two weeks. According to clinicians at OHC, a major benefit is the favorable side effect profile. Unlike conventional chemotherapy and radiation, which SCLC patients typically do not tolerate well, the BiTE therapy has been associated with very few adverse effects.
Patient Outcomes
Jeremy Higgins was 50 years old when diagnosed with small cell lung cancer (搜索) two years ago. Scans revealed multiple tumors in his brain, and the cancer had metastasized to his lungs. After undergoing brain surgery and chemotherapy, he was started on Imdelltra.
"So they brought up the scans and they couldn't see — they couldn't tell that I had ever had a brain tumor," Higgins said of his follow-up imaging. "A year after seeing that brain tumor, I mean, it was really emotional." Higgins has since resumed normal activities including walking, playing golf, and camping with his grandchildren.
Jenny Sirk, another OHC patient, turned to Imdelltra after exhausting other options. "So my only option was to do the treatment, at least try. And going on two years now, so far all my scans have been cleared," Sirk said. She reported losing her sense of taste as a side effect but considers it a small price to pay: "I mean, I can see my children, my grandchildren now, and I do things with them."
Clinical Significance
The responses observed in these patients highlight the potential of BiTE technology to address a critical unmet need in small cell lung cancer (搜索), a disease characterized by rapid progression and poor response to traditional modalities. The ability of Imdelltra to achieve complete responses — including clearance of brain metastases — in patients who had progressed on or exhausted standard therapies underscores its therapeutic promise.
"Being in a position that I'm in now, I have a sense of gratefulness," Higgins said.
