ImmunityBio's CD19 CAR-NK Therapy Achieves 15-Month Complete Response in Waldenström Lymphoma

ImmunityBio has announced promising results from its phase 1 QUILT-106 trial evaluating an off-the-shelf CD19 CAR-NK cell therapy combined with rituximab in patients with Waldenström lymphoma (搜索), a rare B-cell malignancy. The study demonstrated a 100% disease control rate across four evaluable patients who had experienced disease progression following standard-of-care treatment.
Sustained Complete Responses Without Chemotherapy
The most striking finding involved two patients evaluable for long-term follow-up who achieved sustained complete responses of 7 months and 15 months, respectively, after receiving eight doses of the combination therapy with no subsequent treatment. Both patients presented with extensive disease at baseline, including one with multiple lymphomatous bone lesions and another with approximately 95% bone marrow infiltration by tumor cells.
"This updated follow-up reinforces the central thesis that restoring and activating the immune system can deliver durable control of disease without chemotherapy or lymphodepletion," said Patrick Soon-Shiong, MD, founder, executive chairman, and global chief medical and scientific officer of ImmunityBio. Rapid Response in Heavily Pretreated Patients
In the patient with significant bone marrow involvement, complete bone morphological remission was achieved after just four doses of the CAR-NK and rituximab combination. This complete response has been maintained for 15 months and remains ongoing as of the latest data cutoff, despite no further treatment after a total of eight doses.
The findings represent the first chemotherapy-free and lymphodepletion-free immunotherapy regimen combining off-the-shelf allogeneic CD19 CAR-NK cells with rituximab to demonstrate complete disease control in Waldenström non-Hodgkin lymphoma (搜索), administered entirely in the outpatient setting.
Novel Dual-Targeting Mechanism
ImmunityBio's CD19 CAR-NK therapy is engineered as a targeted high-affinity natural killer cell therapy using an off-the-shelf, allogeneic NK cell line. The cells express both a CD19 (搜索)-specific chimeric antigen receptor and a high-affinity CD16 receptor, enabling dual anti-tumor mechanisms: direct CAR-mediated cytotoxicity and augmented antibody-dependent cellular cytotoxicity when paired with rituximab.
This design allows the combination to target both CD19 (搜索)-positive and CD20 (搜索)-positive lymphoma cells, potentially enhancing tumor cell elimination compared to single-target approaches.
Addressing Unmet Medical Need
Waldenström non-Hodgkin lymphoma (搜索) remains an area of significant unmet medical need, particularly for patients who relapse or become refractory to available targeted and antibody-based therapies. The approach eliminates the need for cytotoxic conditioning for lymphodepletion or inpatient hospitalization, addressing key limitations associated with conventional CAR-T therapies.

"To date, all patients have been treated as outpatients with no serious adverse events, demonstrating the feasibility of delivering potent cellular immunotherapy without the morbidity traditionally associated with cell-based treatments."
Trial Design and Future Development
The first-in-human QUILT-106 study enrolled patients with CD19 (搜索)- and CD20 (搜索)-expressing relapsed/refractory B-cell non-Hodgkin lymphoma (搜索). The trial's coprimary endpoints include safety assessment and incidence of treatment-emergent adverse events, while the secondary endpoint focuses on best tumor response per Lymphoma Response to Immunomodulatory Therapy Criteria.
Enrollment and follow-up in QUILT-106 are ongoing, with additional clinical updates planned as more patients become evaluable and response durability continues to mature. A follow-up study is being designed to test the combination of CD19 CAR-NK with ANKTIVA (nogapendekin alfa inbakicept), a superagonist IL-15, and rituximab to build on the success observed in indolent lymphoma including Waldenström's macroglobulinemia.
