IMPACT-TD Registry Reveals Stark Diagnostic Gap: Only 23% of Young Adults with Mood Disorders and Tardive Dyskinesia Symptoms Receive Formal Diagnosis

New data from the IMPACT-TD Registry, the largest study of tardive dyskinesia (搜索) (TD) conducted to date, reveal a striking disconnect between the profound burden of TD symptoms and formal diagnosis rates among younger adults with concomitant mood disorders. The findings, presented by Teva Pharmaceuticals at the 2026 American Psychiatric Association Annual Meeting in San Francisco, show that while 85% of young adults aged 18-29 living with TD movements report moderate to severe impact on their daily lives, only 23% have received a formal TD diagnosis.
The analysis, drawn from the ongoing 3-year, prospective, non-interventional, Phase 4 IMPACT-TD Registry, evaluated 211 adults with TD who were not receiving vesicular monoamine transporter 2 (VMAT2) inhibitor therapy at enrollment and had concomitant mood disorders, including bipolar disorder (搜索) (60%) and depression (搜索) (54%). The broader registry encompasses 611 participants aged 18 years and older.

Age-Stratified Impact and Diagnosis Patterns
The IMPACT-TD findings demonstrated that a majority of participants across all age groups reported moderate to severe global impact from TD. The burden was particularly pronounced among those aged 18-29 (85%, n=13) and those aged 50-59 (87%, n=57), illustrating that TD significantly disrupts daily life throughout the adult lifespan.
Notably, the psychological toll of TD was most severe in adults under 60 years of age. Over three-quarters (77%) of participants aged 18-29 experienced moderate to severe psychological effects, despite having lower average Abnormal Involuntary Movement Scale (AIMS) scores of 6.4, compared to older adults aged 60-69 (8.4, n=56) and those over 69 (9.9, n=28). This finding suggests that the subjective psychological burden does not necessarily correlate with clinician-rated movement severity.
The Diagnostic Gap
Formal TD diagnosis rates were lowest among adults under 40 years old. The rate stood at just 23% for participants aged 18-29 and 35% for those aged 30-39 (n=20), well below the peak of 57% observed in adults aged 40-49 (n=37) and the 47% average across subgroups aged 50 and older.
Compounding this diagnostic gap, the study identified a significant delay in diagnosis. Patients waited an average of more than 3.5 years to receive a formal TD diagnosis after their involuntary movements were first recognized, representing a prolonged period during which individuals may go without appropriate treatment and support.
"Beyond the visible symptoms, tardive dyskinesia (搜索) impacts every aspect of daily living, from personal independence to social interaction and emotional wellbeing," said Richard Jackson, MD, Assistant Clinical Adjunct Professor in the University of Michigan School of Medicine's Department of Psychiatry and IMPACT-TD principal investigator. "What remains a critical unknown is how this debilitating condition uniquely impacts those already struggling with mood disorders, especially at different ages. The IMPACT-TD study is designed to bridge this crucial knowledge gap, giving us the insights we urgently need to offer targeted, meaningful support to every TD patient, no matter their background."
Study Design and Methodology
The IMPACT-TD Registry is a 3-year, prospective, non-interventional, Phase 4 study examining TD progression and its impact on patients' lives. The study includes a broad representation of people affected by TD across age, sex, race/ethnicity, underlying conditions, movement severity, and treatment status. Participants at enrollment were required to have either a score of at least 2 on at least one item of the AIMS with probable TD, or be receiving VMAT2 inhibitor (搜索) therapy for TD. Multidimensional impact was measured using the clinician-reported IMPACT-TD scale, while TD severity was assessed by AIMS.
Tardive dyskinesia (搜索) is a highly debilitating, chronic movement disorder that affects approximately one in four people who take certain mental health treatments. It is characterized by uncontrollable, abnormal, and repetitive movements of the face, torso, and/or other body parts, which can be disruptive and negatively impact individuals' quality of life.
