Internet-Based CBT Shows Promise in Treating OCD During COVID-19 Pandemic
A pilot study conducted at a German psychiatric outpatient department has demonstrated the effectiveness of internet-based cognitive behavioral therapy (ICBT) in treating obsessive-compulsive disorder (OCD) during the COVID-19 pandemic, offering a promising alternative to traditional face-to-face treatment.
The study evaluated the OCD-NET program, a therapist-guided online intervention, between April 2020 and March 2022, enrolling 57 patients with diagnosed OCD. The program consists of 10 modules focusing on exposure and response prevention (ERP), a well-established treatment approach for OCD.
Treatment Outcomes and Effectiveness
The results showed significant improvement in OCD symptoms, with participants experiencing an average reduction of 5.43 points on the Obsessive-Compulsive Inventory-Revised (OCI-R) scale. The treatment achieved a substantial within-group effect size of d=1.13, demonstrating strong clinical impact. Among participants, 29.8% showed treatment response, defined as a 40% or greater reduction in symptoms, while 15.8% achieved remission.
Depressive symptoms, commonly co-occurring with OCD, also showed meaningful improvement with a effect size of d=0.90. Additionally, participants reported increased self-efficacy following treatment completion, suggesting enhanced confidence in managing their condition.
Program Engagement and Satisfaction
The study reported impressive engagement metrics, with 78.9% of participants completing at least four modules - considered the minimum therapeutic dose. Notably, 33.9% of participants completed all ten modules of the program. Patient satisfaction was high, with 98.2% indicating they would recommend the program to others needing similar help.
Pandemic Context and Implementation
The timing of the study during the COVID-19 pandemic provided unique insights into treatment accessibility. Approximately 23.2% of participants cited pandemic-related worsening of OCD symptoms as their reason for seeking treatment, while 21.4% joined due to restricted access to traditional therapeutic services.
Clinical Implications
The program demonstrated particular utility in addressing treatment barriers exacerbated by the pandemic. Participants received regular support from licensed psychologists through the platform, with therapists responding to messages within 24 hours on weekdays. This structured support, combined with the flexibility of self-paced learning, proved effective in maintaining engagement.
The study's findings align with previous international evaluations of OCD-NET, suggesting its potential as a scalable treatment option. The completion rate of 78.9% exceeded several previous ICBT studies for OCD, indicating strong program acceptability.
Future Directions
While these results are promising, researchers note the need for larger randomized controlled trials to definitively establish the program's efficacy. Future studies should include longer-term follow-up assessments and clinician-rated outcomes to complement self-reported measures.
The successful implementation of OCD-NET during pandemic conditions suggests its potential as a valuable addition to the OCD treatment landscape, particularly in situations where access to traditional face-to-face therapy is limited.
