Israeli Patient Detoxes from 130-Pill-a-Day Opioid Addiction in 20 Minutes Using Noninvasive Focused Ultrasound Neuromodulation

One week post-treatment, the patient tested negative for opioids and reported a craving score of zero out of 10, with an unexpected cessation of heavy smoking and alcohol urges.
A team at Rambam Health Care Campus in Haifa has achieved what clinicians are calling a "medical and therapeutic revolution": the complete detoxification of a patient from extreme opioid dependence in just 20 minutes using noninvasive focused ultrasound neuromodulation. The patient, identified only as H., had been consuming approximately 130 painkiller pills daily and was treated while in active withdrawal — a first for the international multicenter study.
H., a 40-year-old family man from northern Israel, sustained a neck injury several years ago and was prescribed opioid painkillers. "Over time the pain lessened, but he could not break free from the dependence on the pills and the doses kept increasing, until they reached a peak of about 130 pills a day," said Dr. Amir Minerbi, director of the Pain Medicine Institute at Rambam. "H. was no longer suffering from pain. He simply needed that substance in his blood so he would be calm and able to function."
How Focused Ultrasound Neuromodulation Works
The experimental procedure, based on technology developed by Israeli company Insightec, intervenes in the electrical activity of the nucleus accumbens (搜索) — the core of the brain's reward system responsible for feelings of satisfaction, pleasure, and reward. Unlike the ablative focused ultrasound already used for essential tremor and Parkinsonian tremor, this new approach performs noninvasive neuromodulation without heating or burning tissue, allowing clinicians to either increase or suppress neural activity in targeted brain regions.
"The new technology allows us to intervene in the brain's electrical activity in a targeted way, and thereby influence focused control areas in brain activity, depending on the disease being treated," explained Dr. Lior Lev-Tov, director of the functional neurosurgery unit in Rambam's neurosurgery division and lead investigator of the study.
The entire procedure was conducted under MRI guidance, enabling precise targeting of deep, sensitive brain structures without the risks associated with invasive surgery.
Immediate and Sustained Results
The clinical outcomes were both rapid and dramatic. "Already during the treatment itself we identified a decrease in the patient's craving for the drug," Dr. Lev-Tov reported. One week after the procedure, laboratory tests returned negative for opioids and other substances. The patient himself reported a craving score of zero out of 10.
An unexpected finding further underscored the treatment's impact on addiction circuitry: H. experienced a drastic reduction in cigarette consumption, dropping from three packs per day to just a few cigarettes, and reported no urge to consume alcohol. "In a treatment that lasted about 20 minutes net, our patient was completely freed from an extreme dependence that had accompanied him every day for years," Lev-Tov said.
"From the moment of treatment until today he has been clean from use of the drug. The urge and craving to use the drug disappeared completely," Lev-Tov added. "He told us that he got his life back. The tests show that the body is completely clean and the patient's physical and functional condition is completely normal."
A Global Epidemic and Current Treatment Limitations
Opioid addiction (搜索) has been recognized as a global epidemic. In the United States alone, it has claimed hundreds of thousands of lives, with economic damage estimated at approximately $60 billion annually. Israel, which several years ago ranked first globally in the rate of growth in opioid use, has since seen the trend halt and begin to decline.
Current detoxification approaches face significant limitations. "Today, detox from painkillers works in two ways: tapering off until stopping use, with success rates of 5%, while even those who manage to quit remain at risk of illness and death afterward," Dr. Minerbi noted. The alternative involves substituting with another drug acting through the same mechanism. Detoxification comprises two stages: physical withdrawal symptom management and psychological detoxification, which can persist for extended periods.
"The new development specifically targets areas in the brain that are responsible for addiction. We hope it will be able to help thousands who are dependent on opioids in a safer and less traumatic way," Minerbi said.
Multicenter Trial and Future Directions
The study is being conducted at three centers in the United States alongside Rambam as part of a multicenter trial that has, according to hospital officials, so far produced excellent results in preserving treatment gains and achieving opioid detoxification. Some patients in the study were addicted to heroin, a substance whose detoxification process can extend for years. H. was the first patient to undergo the treatment while in active withdrawal, a therapeutic challenge that contributed significant insights to the international research effort.
Dr. Lev-Tov emphasized the broader implications: "We are talking about a noninvasive ability to reach very deep and very sensitive areas, as part of a very complex network in the brain that is linked to reward, satisfaction, desire and the ability to control needs and impulses, and we are able to move the range from one end to the other. There is nothing like it in the world of medicine."
He outlined potential future applications: "This experience opens doors for us to treat a wide range of very serious illnesses such as PTSD, OCD, eating disorders, other addictions, severe depression, severe pain disorders, and I hope we will also be able to reach cognitive areas and treat attention deficit disorders, Alzheimer's, Parkinson's, and more."
"Rambam is a global pioneer in the use of FUS technology and has become a center of excellence in the field," Lev-Tov concluded, "one that will continue to push the technology forward and make breakthrough treatment accessible to patients in additional indications."
