Ivonescimab (SMT112), a novel bispecific antibody targeting both VEGF (搜索) and PD-1 (搜索) pathways, is showing promise as a potential treatment for patients with clear cell renal cell carcinoma (搜索) (ccRCC) who have developed resistance to current therapies. The investigational agent is being evaluated in the phase 2 IVORY trial (NCT06940518) for patients with advanced ccRCC previously treated with immune checkpoint blockade.
Addressing Treatment Resistance in RCC
For years, patients with RCC (搜索) have been treated with two main therapeutic approaches: VEGF (搜索)-directed therapies targeting angiogenesis and immunotherapies addressing immune escape mechanisms. While these treatments have demonstrated significant progress in the first-line setting, resistance development remains inevitable for most patients.
"For so long, we have been treating our patients with RCC (搜索) with 2 main regimens. One is directed towards angiogenesis by VEGF (搜索)-directed therapies, and the other one is directed towards immune escape—immunotherapies," explained Nazli Dizman, MD, a hematology/oncology fellow at MD Anderson Cancer Center, who presented data from the IVORY trial at the 2025 Kidney Cancer Research Summit.
Dual Pathway Inhibition Strategy
Ivonescimab represents a novel approach by simultaneously inhibiting both pathways within the same therapeutic agent. As a bispecific antibody, it targets both VEGF (搜索) and PD-1 (搜索) pathways in close proximity, potentially offering advantages over sequential or combination therapies.
"Ivonescimab is a novel drug that inhibits both [pathways] at the same time in the same place. That sparked our attention in studying this agent in the immunotherapy resistance setting for RCC (搜索)," Dizman noted.
The simultaneous and proximal inhibition of both major pathways involved in RCC (搜索) pathogenesis may provide broader coverage to address tumor heterogeneity, a key challenge in treating resistant disease.
Preclinical Evidence and Clinical Promise
Preclinical studies have provided evidence supporting the synergistic potential of dual VEGF (搜索) and PD-1 (搜索) inhibition using a bispecific methodology. This mechanistic approach may lead to improved outcomes compared to traditional sequential treatments.
The agent has demonstrated superior efficacy in lung cancer (搜索) trials compared to pembrolizumab (Keytruda), suggesting its potential applicability across different cancer types. "This agent has shown impressive superior outcomes with ivonescimab in lung cancer over pembrolizumab, so there is a possibility that we could utilize that performance in kidney cancer," Dizman explained.
Favorable Safety Profile
Ivonescimab exhibits a favorable toxicity profile with a short half-life, potentially offering advantages in terms of safety and tolerability. The agent may demonstrate lower rates of treatment discontinuation due to adverse effects compared to other standards of care.
"Toxicity-wise, also, it seems to have a favorable profile, similar to what we would expect from the combination of these 2 mechanistic strategies administered in 2 different ways, but potentially lower rates of treatment discontinuation due to adverse [effects]," Dizman observed.
Contributing to RCC Treatment Evolution
The development of ivonescimab represents part of the continued momentum in kidney cancer therapeutic advancement. "My generation in kidney cancer has been spoiled with the development of new therapies in this disease in the past 10 years. Ivonescimab could potentially serve to this momentum that we keep seeing new therapies coming in for kidney cancer," Dizman concluded.
The IVORY trial continues to evaluate this dual-pathway inhibition approach in patients with advanced ccRCC who have previously received immune checkpoint blockade therapy, potentially offering a new treatment option for this challenging patient population.
