Clarkson confirmed that he underwent surgery to remove a portion of his prostate. In a discussion with Cooper, he stated: "The prostate, 10% of it is dead, the 10% where the cancer is." He later told co-star Gerald Cooper: "I had the op, and just fingers crossed it's worked, we don't know yet."
Despite his initially upbeat outlook, the series concluded with Clarkson being rushed to hospital after experiencing complications. "Some of the treatment's gone a bit awry, let's say, so I'm going to be here for a little while. I'm nil by mouth, I don't know what's going to happen," he said from his hospital bed. He added: "But if this is all successful, I'll see you for season six, and if it isn't, I won't."

This latest diagnosis follows another significant health event for Clarkson. The fifth series of Clarkson's Farm opened with him discussing a heart procedure he underwent in October 2024, after experiencing tightness in his chest.
