Johnson & Johnson Beats Q2 Estimates, Raises Full-Year 2026 Guidance on Strong Pharmaceutical Sales

Johnson & Johnson delivered stronger-than-expected second-quarter results and raised its full-year 2026 outlook, powered by robust pharmaceutical sales that more than offset headwinds from products facing patent competition. The healthcare giant posted adjusted earnings per share of $2.90, ahead of the Wall Street consensus estimate of $2.85, while revenue rose 6.6% year-over-year to $25.31 billion, exceeding analysts' expectations of $25.05 billion.
Net earnings for the quarter were $5.53 billion, or $2.27 per diluted share, compared with $5.54 billion, or $2.29 per share, in the prior-year period. Adjusted net earnings increased 5.7% to $7.08 billion.
Innovative Medicine Leads Growth
The Innovative Medicine segment remained the primary growth driver, with sales rising 7.8% to $16.38 billion, above expectations of approximately $16.1 billion. MedTech sales increased 4.5% to $8.93 billion.
Among individual products, the immunology drug Tremfya generated $2 billion in sales, up 72.5% from a year earlier and well above analyst estimates of $1.74 billion. The cancer therapy Darzalex also contributed meaningfully to growth, helping offset the impact of patent expirations affecting older medicines such as Stelara.
Spravato Momentum Signals Broader Psychedelic Drug Opportunity
Johnson & Johnson's Spravato (esketamine nasal spray) posted worldwide second-quarter sales of $584 million, up 25% from the first quarter, including U.S. sales of $514 million. According to Jefferies analysts, these figures imply an annualized revenue run rate of more than $2.3 billion and keep the product on track to reach the company's previously guided annual sales of $3 billion to $3.5 billion by 2027–2028.
Jefferies wrote that Spravato's momentum in treatment-resistant depression (搜索) and major depressive disorder (搜索) with suicidal ideation "supports the notion psychedelics can be commercially viable in hard-to-treat mental health disorders," citing Johnson & Johnson's investment in treatment sites, physician education, and reimbursement infrastructure. The firm noted that J&J has guided for Spravato peak annual sales of up to $5 billion and estimated that more than 250,000 patients worldwide may have been treated with the therapy cumulatively.
The analysts also pointed to increasing investor interest in psychedelic medicines following positive mid- and late-stage clinical trial data across depression, anxiety, and post-traumatic stress disorder, alongside an improving regulatory backdrop and growing interest from large pharmaceutical companies. J&J's network of approximately 7,000 to 8,000 U.S. treatment sites could help facilitate future adoption of psychedelic therapies, with Jefferies noting that Atai's intranasal BPL-003 could fit within the same two-hour treatment framework currently used for Spravato.
Raised Full-Year 2026 Guidance
Following the stronger-than-expected quarter, Johnson & Johnson increased its full-year 2026 guidance. The company now expects reported sales of $100.8 billion to $101.4 billion, with a midpoint of $101.1 billion, and adjusted earnings per share of $11.60 to $11.75, compared with its previous outlook. The updated adjusted EPS guidance has a midpoint of $11.68.
Johnson & Johnson also reported year-to-date free cash flow of approximately $8.7 billion, up from $6.21 billion in the prior-year period.
CEO Highlights Innovation Pipeline
Joaquin Duato, Johnson & Johnson's CEO, highlighted the company's innovation pipeline, noting recent approvals for Tremfya in active psoriatic arthritis (搜索), Caplyta for preventing relapse in schizophrenia (搜索), and the Dual Energy ThermoCool SmartTouch SF platform, alongside positive clinical data for several oncology and surgical programs.
"With raised guidance and quarterly sales surpassing $25 billion, we are on track to meet our 2026 target of more than $100 billion in annual revenue for the first time in our Company's 140-year history," Duato said.
Shares of Johnson & Johnson were little changed at $253 following the release of its earnings.
