Large-Scale Danish Study Finds No Link Between Aluminum in Childhood Vaccines and Chronic Diseases

A comprehensive nationwide study of more than 1.2 million Danish children has found no association between aluminum exposure from childhood vaccines and the development of chronic diseases, providing robust evidence for vaccine safety amid growing skepticism about immunization programs.
The research, published in the Annals of Internal Medicine, examined the relationship between cumulative aluminum exposure from vaccines administered during the first two years of life and the incidence of 50 chronic health conditions. The study represents one of the largest investigations into aluminum adjuvant safety, covering children born in Denmark between 1997 and 2018 who were followed through the end of 2020.
Study Design and Scope
The cohort study included 1,224,176 children who received aluminum-containing vaccines during their first two years of life. Researchers focused on three major categories of chronic conditions: 36 autoimmune disorders (including dermatologic, endocrinologic, hematologic, gastrointestinal, and rheumatic conditions), nine atopic or allergic conditions (asthma, atopic dermatitis, rhinoconjunctivitis, and allergies), and five neurodevelopmental disorders (autism spectrum disorder and attention deficit-hyperactivity disorder).
"Our results, based on 1.2 million Danish children and a study period covering 24 years, provide robust evidence supporting the safety of childhood vaccines," said Anders Hviid, MSc, DrMedSci, co-author from Statens Serum Institut. "This is evidence that parents, clinicians, and public health officials need to make the best choices for the health of our children."
Key Findings
The analysis revealed no increased risk for any of the 50 health conditions examined in relation to aluminum exposure from vaccines. For each 1-mg increase in aluminum exposure, the adjusted hazard ratios were 0.98 (95% CI, 0.94–1.02) for autoimmune disorders, 0.99 (95% CI, 0.98–1.01) for atopic or allergic conditions, and 0.93 (95% CI, 0.90–0.97) for neurodevelopmental disorders.
For most individual conditions, the upper limits of the 95% confidence intervals did not support relative risk increases exceeding 10% to 30%. The researchers concluded that findings were "incompatible with moderate to large relative increases in the risks for autoimmune, atopic or allergic, and neurodevelopmental disorders associated with early childhood exposure to aluminum-adsorbed vaccines for most outcomes, although small relative increases could not be statistically excluded, particularly for some rarer outcomes."
Aluminum Adjuvants in Vaccines
Aluminum-containing adjuvants are incorporated into non-live vaccines to strengthen immune response by attaching to vaccine antigens. These adjuvants are found in several routine childhood immunizations, including vaccines for diphtheria, tetanus, and pertussis; Haemophilus influenzae type b (Hib); pneumococcal conjugate (PCV); and hepatitis A and B.
"An adjuvant is a substance that alerts the body's immune response to the vaccine's antigen," explained Ross Kedl, a vaccine expert at the University of Colorado Anschutz Medical Campus (搜索) who reviewed the findings. "Without adjuvants, you actually create tolerance, which is the opposite effect of what you want a vaccine to do."
Addressing Safety Concerns
The study was motivated by ongoing concerns about potential harms of aluminum-adsorbed vaccines, particularly following a 2022 CDC (搜索)-funded study that suggested a link between aluminum-containing vaccines and asthma. However, that earlier study has been widely criticized for failing to distinguish aluminum in vaccines from aluminum from other sources such as food, water, air, and breast milk.
"Aluminum is part of our daily diet and has been since the beginning of time. That is the point people don't understand," said Michael Osterholm, director of the Center for Infectious Disease Research and Policy at the University of Minnesota.
Hviid emphasized the distinction between aluminum salts used in vaccines and elemental aluminum: "The aluminum that is in vaccines is in the form of extremely small amounts of aluminum salts, which is not the same as elemental aluminum which is a metal. It's really important for parents to understand that we are not injecting metal into children."
Clinical Implications
The research comes at a time of heightened vaccine skepticism in the United States, with concerns about aluminum adjuvants joining other vaccine safety debates. Most aluminum from vaccines leaves the body within two weeks, though trace amounts can remain for years.
"One study does not make for a safe vaccine supply or not," Osterholm noted. "It's the accumulative data that comes from many studies that have been done, that together demonstrate the safety of vaccines."
The Danish study's unique strength lies in the country's unified health system, which allows researchers to track individuals from birth throughout their lives. "This excellence is partly because they have, for a long time, had such a unified health system," Kedl explained. "Everyone is tracked for life from birth and you can go back for many years and ask, 'Can we find a link between something that happened in the past and in the future?'"
Hviid stressed the importance of maintaining scientific objectivity in vaccine discussions: "It is critically important that we keep politics and science apart in this issue. If not, it is the children, including U.S. children, who are going to suffer the consequences."
