The findings, drawn from 2,410 participants in the Framingham Heart Study (搜索) with a mean age of 70 (±8.45 years, 55.2% female), employed restricted cubic splines (RCS) — a non-linear modeling technique — to uncover a relationship that conventional linear analyses would have missed entirely. "When you force a straight line through data that curves, you miss the story entirely," said Vanessa M. Young, PhD, MS, first and corresponding author and a postdoctoral research fellow at the Glenn Biggs Institute for Alzheimer's and Neurodegenerative Diseases at UT Health San Antonio. "We knew from the existing literature that the relationship between sleep and these biomarkers was unlikely to be simple, and that's exactly what we found."
The 8.5-Hour Inflection Point
The RCS analysis revealed a robust non-linear association between self-reported sleep duration and plasma p-tau181 (搜索) (overall p = 0.005; non-linearity p = 0.002). Plasma p-tau181 levels began accumulating noticeably once regular sleep durations reached 8.5 to 9 hours per night, with the risk curve spiking dramatically upward beyond the 10-hour threshold. Notably, categorical comparisons using standard cutoffs (≤6 hours, >6–<9 hours as reference, ≥9 hours) showed no association in adjusted models, underscoring the necessity of non-linear approaches to detect this signal.
The association withstood rigorous covariate adjustment for age, sex, sleep apnea, depression, kidney function, and apolipoprotein E ε4 genotype — the common genetic variant linked to increased late-onset Alzheimer's risk. "The non-linear association held firm even after the mathematical models were heavily adjusted to rule out an array of confounding health factors," the researchers noted.
Specificity of p-tau181 (搜索) Among Biomarkers
The research team simultaneously tracked four blood-based proteins linked to cellular damage and neurodegeneration: p-tau181 (搜索), total tau, neurofilament light chain (NfL), and glial fibrillary acidic protein (GFAP). For the latter three markers, the sleep association disappeared once kidney function was accounted for. Only p-tau181 withstood the adjustment, suggesting the link between long sleep and this particular protein may be specific to Alzheimer's-related processes. The authors caution, however, that more research is needed to confirm this specificity.
A Behavioral Footprint, Not a Cause
Young explicitly clarified the study's interpretive boundaries. "Because this is a snapshot in time rather than a long-term study, we cannot say that long sleep causes Alzheimer's, but the findings suggest it may be worth monitoring, and that more sleep is not always better for brain health." The cross-sectional design means long sleep cannot be labeled as a direct cause of Alzheimer's disease (搜索). Instead, excessive sleep is highly likely to be a downstream behavioral symptom — an early physiological reaction as the brain attempts to cope with initial, silent neurodegenerative changes.
The study builds on a 2025 investigative milestone by the same research group, which demonstrated that sleeping nine hours or more per night was significantly associated with worse cognitive performance, particularly among individuals managing co-occurring depression. The current analysis extends that work by incorporating blood-based biomarkers and more sophisticated modeling.
Clinical Implications and Public Health Context
Dementia affects 57 million individuals globally, with Alzheimer's disease (搜索) accounting for 60% to 70% of cases. Even with recent advances in disease-modifying therapies, Alzheimer's remains a profound medical and societal challenge, making the identification of modifiable risk factors a critical research priority.
The public health takeaway translates into a practical diagnostic boundary. "In plain terms, if you regularly find yourself sleeping nine to 10 hours or more a night, it may be worth mentioning to your doctor as a useful conversation starter about your sleep quality and overall brain health," Young suggests. The researchers concluded that the findings "identify long sleep as a potential behavioral marker of elevated p-tau181 (搜索), with implications for risk assessment that warrant prospective validation."
Longitudinal studies are now needed to clarify temporal relationships and mechanistic pathways linking sleep duration to Alzheimer's pathology.
