Long-Term Antidepressant Users May Need Slower, Hyperbolic Tapers as Withdrawal Concerns Grow

A new Nature feature has reignited scientific debate over how long-term antidepressant use should be discontinued, arguing that current tapering guidance may move too fast for patients who have taken the drugs for years.

Psychiatrist Mark Horowitz spent 15 years taking an SSRI (搜索) antidepressant. It took him five years, and a much slower, more gradual approach, to finally come off it. His experience, detailed in a new feature published by Nature, has reignited scientific debate over how long-term antidepressant use should be discontinued, and whether current tapering guidance moves too fast for people who have been on the drugs for years.
Why Duration Matters
Horowitz, a psychiatrist at Adelaide University in Australia, argues that treatment duration is one of the strongest predictors of how difficult withdrawal becomes. "The longer you're on the drugs, the more your brain adapts, and the more severe withdrawal is," he said.
That view has gained traction among some clinicians, even as it remains contested. Maurizio Fava, a psychiatrist at Massachusetts General Hospital, has separately acknowledged that withdrawal is "a real phenomenon," though some clinical trialists contest how common or severe it typically is.
The Neurobiology of Withdrawal
Dr. Bryan Shapiro, MD, a board-certified psychiatrist at UC Irvine Medical Center and Senior Medical Advisor at Outro (搜索), a company that specializes in clinician-guided tapering, explains the underlying mechanism. "Selective serotonin reuptake inhibitors (SSRIs) like Zoloft or Lexapro increase brain serotonin levels, and in response, serotonin receptors become less sensitive over time (a process called downregulation)," he says. "So when the medication disappears suddenly, it is extremely jarring to the brain because not only are serotonin levels suddenly reduced, but the serotonin receptors are downregulated and generally require several, several weeks to return to their baseline functioning."
Dr. Cooper Stone, DO, Clinical Assistant Professor of Psychiatry and Behavioral Sciences at the University of Pennsylvania, calls this a "shock" to your body.
A Widely Debated Risk
Estimates of how many people experience withdrawal symptoms vary enormously depending on the type of study. A 2019 analysis found that 56 per cent of people who tried to stop taking SSRIs experienced discontinuation symptoms, with nearly half describing them as severe.
Controlled trials from more recent years suggest a much lower figure, closer to 15 per cent, according to the Nature report. But many of those trial participants had only taken the drugs for a few weeks or months, far shorter than the years or decades typical of real-world use, which may explain the gap.
According to trial data cited in the Nature feature, roughly half of people who stop taking antidepressants relapse within a year. This is one reason most patients are advised to stay on the medication for at least six months after their symptoms stabilise.
The Slow Taper Revolution NICE Is Already Backing
UK guidance has already begun shifting towards slower, more individualised tapering schedules. The National Institute for Health and Care Excellence (搜索) updated its recommendations to favour reductions that get smaller as the dose gets lower, an approach researchers call "hyperbolic" tapering, according to a 2024 analysis in the British Journal of General Practice for primary care doctors.
"Hyperbolic tapering is based on the simple idea that cutting your drug dose by 50% doesn't reduce its effect by 50%," Dr. Shapiro says. "In other words, the relationship between dose and brain activity isn't linear. We learned about this counterintuitive pattern from brain imaging data that emerged in the early 2000s, which directly measured the effects of serotonin-based antidepressants on their receptors in the brain, and hyperbolic tapering is the product of this idea."
Dr. Shapiro adds that hyperbolic tapering involves reductions designed to provide a more "even" reduction in biological effects at each step, which is much less disruptive and considerably reduces the risk of withdrawal symptoms. "Historically, tapers were done 'linearly,' and patients were stopped at doses that seemed very low, but still had considerable biological activity," he says. "This is why withdrawal syndromes have been so common."
A separate case study published in The Pharmaceutical Journal noted that around half of patients experience some withdrawal effects, and that symptoms can, in some cases, last for months.
Withdrawal Symptoms and Timeline
With the exception of the antidepressant fluoxetine, which stays in the body for up to several weeks at a time, patients generally begin to feel symptoms of antidepressant withdrawal within a few days of stopping their antidepressant, according to Dr. Stone. Symptoms can include full body muscle aches, electric shock-like sensations, tremors, abnormal skin sensations such as numbness and tingling, dizziness, headaches, insomnia, irritability, nausea, anxiety, dysphoria, sweating, chills, and fatigue.
Dr. Shapiro highlights a distinctive neurologic symptom unique to SSRI (搜索) withdrawal: "There is an interesting neurologic symptom that is unique to withdrawal from SSRI antidepressants called 'brain zaps.' It's an electric-shock-like sensation that feels like it's coming from your brain, even though your brain has no sensory or pain receptors."
"Generally, these symptoms will slowly resolve over the course of several weeks, but it's possible it can go on for months at a time," Dr. Stone says.
Distinguishing Withdrawal from Relapse
When it comes to telling the difference between antidepressant withdrawal during tapering and a full-on depression (搜索) relapse, Dr. Shapiro says the timeline is a very important factor. "If mood symptoms occur within four weeks of stopping an antidepressant very quickly, we would generally chalk that up to withdrawal," he says. "We also consider whether the symptoms resemble the original condition for which the antidepressant was started. Mood symptoms that are accompanied by new bodily symptoms, such as sudden dizziness, nausea or 'brain zaps,' is a clear indicator of withdrawal."
Toward Personalized Tapering Plans
Dr. Stone emphasizes that communication is key when devising a tapering plan with a doctor, describing a "shared decision-making model." "Tapers are generally more successful when patients actively participate in deciding the pace of the taper and the method of the taper," he notes. "There should not be a rigid, set schedule for the taper; rather, it needs to be dynamic and adapt to how the patient responds."
Dr. Shapiro advises patients to ask their clinician open-ended questions about their deprescribing approach, including whether they are aware of hyperbolic tapering, what they would recommend if withdrawal symptoms emerge, and what the lowest dose would be before completely stopping the antidepressant.
Limitations and Unmet Needs
Some imaging and observational studies support the idea that the slower hyperbolic approach eases symptoms, though no randomized trial has proven it safer or more effective than a standard faster taper. Hyperbolic tapering can also be logistically difficult, since it often requires liquid formulations or customised dosing that standard pill sizes don't allow for.
More than 100 million prescriptions for common SSRIs, including sertraline, escitalopram and fluoxetine, were filled in the United States in 2024 alone. With so many people on long-term treatment, even a modest proportion struggling to stop represents a significant number of patients. For readers currently taking antidepressants, the emerging consensus isn't that the drugs are unsafe. It's that stopping them, particularly after years of use, may require more time, planning and medical support than standard tapering schedules currently allow.
