Long-Term TORCH Surveillance Reveals Persistent Congenital Infection Risks in North India Despite Vaccination Gains

A six-year analysis of laboratory records from a major tertiary care teaching hospital in North India has revealed that TORCH pathogens continue to pose a significant threat to fetal health, even as vaccination efforts against rubella (搜索) show measurable success. The findings, presented at ASM Microbe 2026 in Washington, D.C., highlight the need for sustained, pathogen-specific public health strategies in resource-limited settings.
The study was led by Sudhahar Tamizhan, M.D., a physician at the All India Institute of Medical Sciences (AIIMS), New Delhi, with microbiologist Suneeta Meena, M.D., serving as senior author. Microbiologist Rojaleen Das, M.D., also collaborated on the research.
"We hope this work contributes to more informed public-health policies and better prevention of congenital infections in India and other resource-limited settings," Tamizhan said.
The TORCH Complex and Its Clinical Significance
The TORCH complex (搜索) encompasses pathogens responsible for toxoplasmosis (搜索), rubella (搜索), cytomegalovirus (搜索) (CMV), herpes simplex virus (搜索), and other infections. While these pathogens typically cause mild or even silent symptoms in expecting mothers, they can lead to devastating outcomes in a developing fetus, including stillbirth, lifelong disability, and birth defects.
Despite the severity of these outcomes, reliable surveillance data on TORCH infections in India have remained limited, creating a critical gap in evidence needed to shape prevention policies.
Study Design and Key Findings
Tamizhan and colleagues analyzed lab records from individuals tested for TORCH pathogens between 2019 and 2025 at a single tertiary care teaching hospital. The researchers evaluated antibody patterns associated with exposure or immunity to the pathogens, uncovering distinct epidemiological trends.
CMV exposure remained constant throughout the entire study period, demonstrating that the virus circulates widely in the community. The authors noted that CMV's high prevalence highlights the need for greater awareness of this infection, which often receives less attention than other TORCH pathogens.
In contrast, the researchers found consistently high immunity to rubella (搜索) among patients. Rubella is preventable by vaccination, and the data suggest that immunization initiatives in the region are having a positive impact. However, a significant minority of patients remained susceptible—not only to rubella but also to toxoplasmosis (搜索), which is preventable through safe food preparation, hand washing, and careful handling of cat litter.
Pathogen-Specific Prevention Strategies Needed
The divergent epidemiological patterns suggest that different TORCH pathogens require distinct prevention approaches. "A 'one-size-fits-all' approach to TORCH testing may not always be the most effective strategy," Tamizhan said.
While maintaining high vaccine coverage remains important for rubella (搜索) prevention, public health education regarding food hygiene, environmental exposure, and prenatal care may help reduce the risk of toxoplasmosis (搜索) and other congenital infections. Studying these TORCH microbes collectively, Tamizhan explained, gives researchers broad perspectives on infectious risks and exposure patterns, which can inform strategies for prevention.
"Our findings support the importance of sustained surveillance, targeted public health education and evidence-based screening approaches tailored to regional epidemiological patterns," Tamizhan added.
Limitations and Future Directions
Tamizhan cautioned that the new findings represent observations from a single-center, retrospective study. Larger, multicenter investigations are needed to better understand regional variations and evolving trends in congenital infection risk across India.
The research team is now planning follow-up studies to explore age-specific and pregnancy-related risk factors, with particular emphasis on improving awareness and early identification of CMV and other congenital infections.
