Lung Cancer Is No Longer Just a Smoker's Disease: The Case for Broader Awareness and Earlier Detection

For decades, lung cancer (搜索) has been closely associated with cigarette smoking. While tobacco remains the single biggest risk factor, that long-held perception is becoming increasingly incomplete. A growing number of patients diagnosed with lung cancer today have either never smoked or quit smoking many years earlier, a shift that calls for a broader public conversation about who is at risk and why.

A Multifactorial Disease Beyond Tobacco
There is no single explanation for why lung cancer (搜索) develops in people who have never smoked. In many cases, it is likely the result of multiple factors acting over time. Regular exposure to second-hand smoke remains a recognised risk, while worsening air quality and prolonged exposure to fine particulate pollutants are increasingly being linked to lung disease and cancer. These risks often build silently over many years, without causing any obvious symptoms.
In many parts of India, indoor pollution from biomass fuels used for cooking continues to expose women to harmful smoke over several years. Occupational exposure to substances such as asbestos, silica, diesel exhaust and radon gas can further increase the risk in certain individuals.
In addition, inherited genetic changes and acquired molecular mutations have an important role, particularly among younger patients and women who have never smoked. Many of these tumours harbour targetable genetic alterations, making molecular testing an essential part of treatment planning.
Recognising Vague Early Symptoms
Lung cancer (搜索) often produces vague symptoms in its early stages. A persistent cough, chest pain, unexplained breathlessness, coughing up blood, recurrent chest infections or unintentional weight loss should not be dismissed simply because an individual has never smoked. These symptoms warrant timely medical evaluation, especially when they persist beyond a few weeks or fail to respond to routine treatment.
Dr Anvesh Dharanikota, HOD-Surgical Oncology at Arete Hospitals (搜索), emphasises that advances in diagnosis and treatment have significantly changed the outlook for many patients. High-quality imaging, minimally invasive biopsy techniques, comprehensive molecular profiling and precision therapies have helped improve outcomes in selected groups. Immunotherapy has also transformed treatment for many patients with advanced disease. However, these advances can only be fully utilised when the disease is detected early and accurately characterised.
The Challenge of Late Detection
The majority of lung cancer (搜索) patients are detected at a late stage, when chances of survival plummet and treatment costs soar. Several challenges contribute to this diagnostic delay. Low awareness of lung cancer symptoms leads to a lag between symptom onset and the first visit to a clinician. Often, early lung cancer symptoms are mild and dismissed as respiratory issues. In India, which is a tuberculosis (搜索)-endemic country, the symptom overlap often leads to lung cancer being misdiagnosed as tuberculosis.
Certain symptoms such as coughing of blood (haemoptysis) and a persistent cough lasting more than six weeks are red flags that require immediate referral to a specialist. Timely detection requires both effective screening and increased awareness of symptoms, which include cough, loss of appetite, weight loss, fatigue, chest pain and hoarseness of voice.
Screening Barriers and Emerging Solutions
The World Health Organization highlights the importance of screening high-risk individuals for early lung cancer (搜索) detection. However, in low- and middle-income countries like India, health system barriers such as limited access to facilities, a dearth of trained personnel, and financial constraints hinder implementation of effective and equitable lung cancer screening, leading to late detection.
Low-dose computed tomography (LDCT) is proven effective for lung cancer (搜索) screening in high-risk individuals. Yet in the Indian context, nation-wide LDCT-based screening faces several challenges. Radiological similarities with tuberculosis (搜索) lead to high false positive rates, although recent evidence suggests LDCT could be effective in tuberculosis-endemic regions. Availability of LDCT equipment and trained professionals is limited, and an LDCT machine can cost up to ₹2 crore, creating a significant barrier to establishing such facilities, particularly in non-metro cities and rural areas. The per-test cost of nearly ₹7,000 is unaffordable in India, where half or more of healthcare expenses are still paid out-of-pocket.
Chest X-rays, with 77–80% sensitivity, cost up to ₹300 but miss 90% of lung cancer (搜索) cases. However, AI integration and training can potentially improve accuracy and distinguish lung cancer from tuberculosis (搜索). Portable AI-powered X-rays can boost screening in rural India, where 70% of the population lives and tobacco use is higher (as per NFHS-5), and can serve as an interim solution until LDCT infrastructure improves.
Smoking Cessation Remains Central
At the same time, smoking cessation remains one of the most effective ways to reduce the burden of lung cancer (搜索). The message is not that smoking is no longer dangerous; on the contrary, tobacco continues to account for the majority of lung cancer cases and increases the risk of several other cancers and chronic illnesses. The conversation simply needs to become more inclusive.
As per the National Family Health Survey-5, 39% of men and 4% of women in India are smokers. Smoking shortens life by almost 10–11 years. In India, lung cancer (搜索) is the second-leading cause of cancer-related deaths in men and the fourth in women. Beyond cancer, smoking dramatically elevates the risk of numerous other serious health conditions, including stroke and cardiovascular diseases. In 2017–18, tobacco use imposed an economic cost of ₹1.77 lakh crore (1.04% of India's GDP), with smoking accounting for 74% of these costs.
Quitting smoking is hard, taking numerous attempts, and only a fraction of users succeed. Smoking cessation often fails because most attempts are made without behavioral support, which significantly improves success rates. Cognitive Behavioural Therapy (CBT) helps individuals recognise and reshape the thought patterns driving their smoking habits, and research shows that combining CBT with other cessation supports—including nicotine replacement therapy, exercise and financial incentives—significantly boosts success rates. To make cessation scalable, almost 400 mobile apps, including one from the WHO, have been developed, though only a few provide evidence-based support. In India, the National Tobacco Control Programme (搜索) offers free support for smoking cessation.
Reducing Stigma and Broadening the Conversation
Public awareness should move beyond the misconception that lung cancer (搜索) affects only smokers. Equally important is removing the stigma that often surrounds the diagnosis. Patients who have never smoked frequently face disbelief, while smokers may hesitate to seek care because of guilt or fear of being judged. Neither response serves the interests of early diagnosis or effective treatment.
Lung cancer (搜索) is ultimately a disease influenced by multiple factors, including environmental exposures, genetics and lifestyle. Recognising this changing reality encourages earlier evaluation of symptoms, supports timely diagnosis and ensures that more patients receive appropriate treatment when it can make the greatest difference. In the absence of accessible and affordable lung cancer screening and effective anti-tobacco policies, quitting smoking remains the most effective and individually responsible measure to prevent lung cancer—and timely smoking cessation reverses much of the damage.
