Luxturna Gene Therapy Shows Functional Benefits Despite Structural Decline in Real-World Study

The findings, published in the British journal Eye, provide important insights into the long-term effectiveness of this FDA-approved gene therapy.
Study Design and Patient Population
The single-center prospective study, conducted between May 2021 and February 2025, analyzed outcomes in 12 patients (24 eyes) with a mean age of 25.9 years. All patients received bilateral treatment with the AAV2-based gene augmentation therapy, which delivers a functional RPE65 (搜索) gene to address mutations that cause early-onset inherited retinal dystrophies (搜索).
Eligible patients had confirmed biallelic pathogenic or likely pathogenic RPE65 (搜索) variants, evidence of viable retinal cells on spectral-domain OCT with central foveal thickness greater than 100μm, best-corrected visual acuity better than 20/800, and the ability to complete functional testing.
Functional Improvements in Light Sensitivity
The most significant therapeutic benefit emerged in light sensitivity measurements. Full-field stimulus testing demonstrated improvement in median thresholds from -2.2 at baseline to -3.9 log(cd.s/m²) after two years. This enhanced light sensitivity correlated strongly with reduced disability in mesopic peripheral function (r = 0.767, p = 0.016) and decreased rod-function anxiety (r = 0.667, p = 0.050).
These functional improvements translated into meaningful quality of life benefits for patients, particularly those under 30 years of age who achieved superior outcomes compared to older patients.
Mixed Visual Acuity Results
While light sensitivity improved, visual acuity outcomes were more complex. Best-corrected visual acuity declined by a median of 6 ETDRS letters from baseline, dropping from 55.5 [41.2; 69.2] to 36.0 [23.8; 58.0] (p = 0.030). However, visual field measures, including mean deviation and foveal sensitivity, remained essentially stable throughout the 24-month follow-up period.
Structural Concerns and Safety Profile
Multimodal imaging revealed concerning structural changes, with progressive retinal alterations consistent with treatment-associated degeneration. The researchers observed chorioretinal atrophy appearing at injection sites, in treatment areas, and in the retinal periphery, suggesting distinct pathophysiological mechanisms.
"Since multimodal imaging revealed new atrophic changes and/or progression of preexisting ones, this suggests that VN may not fully halt retinal degeneration," the researchers explained. They noted that while chorioretinal atrophy did not appear to compromise functional improvement, "the potential for atrophic lesion enlargement with subsequent foveal involvement raises concerns about its long-term outcomes."
The safety profile aligned with previous reports, with at least one adverse event recorded in 91.7% of eyes. Most adverse events were considered mild and occurred within the first postoperative months. Importantly, no severe complications such as retinal detachment or endophthalmitis were observed. Foveal thinning and progression of preexisting atrophy were noted in some patients, which researchers attributed to the severity of the underlying disease.
Age-Related Treatment Response
A key finding was the superior response in younger patients. Those under 30 years of age achieved better functional outcomes than patients 30 years or older, underscoring the importance of earlier intervention in RPE65-associated retinal degeneration (搜索).
Clinical Implications
The study results highlight both the promise and limitations of Luxturna therapy. While patients experienced meaningful improvements in light sensitivity and quality of life, the progressive structural changes and variable visual acuity effects raise important questions about long-term outcomes.
