MANIFEST IO Launches Landmark UK-Wide Prospective Study to Unravel Immunotherapy Response and Toxicity

The MANIFEST IO (搜索) prospective study has recruited its first patient, marking a major milestone for the UK-wide platform launched in Q4 2024.
The study aims to recruit 3,000 patients across the UK starting immunotherapy (搜索) for breast, bladder, kidney cancer (搜索), and melanoma (搜索).
Researchers will perform deep clinical and biological profiling to understand why some patients benefit more from immunotherapy (搜索) and why toxicities occur.
The first patient has been recruited to a landmark prospective study that aims to decode the biological underpinnings of immunotherapy (搜索) response, resistance, and toxicity. Led by The Christie NHS Foundation Trust, the MANIFEST study represents one of the largest initiatives of its kind in the United Kingdom, with plans to enroll 3,000 patients across the country who are starting immunotherapy for cancers including breast, bladder, kidney, and melanoma (搜索).
The milestone was shared by Jonathan Lim, Associate Faculty at Cancer Research UK Manchester Institute, Deputy Lead of the MANIFEST Consortium, and Lead of the RISE Consortium at RISE Manchester. "Launching our prospective recruitment represents a significant milestone since we launched in Q4 2024. In doing so, we will have the opportunity to learn from every patient," Lim stated.
A Platform Built on Deep Profiling
The MANIFEST IO (搜索) platform is designed to provide deep profiling of patients receiving immunotherapy (搜索), enabling researchers to investigate the biological mechanisms that govern differential treatment responses. By following patients longitudinally throughout their treatment course and studying how their immune systems evolve over time, the study aims to build a comprehensive picture of the factors that drive both therapeutic benefit and immune-related adverse events.
Dr Marija Miletić, MD, Senior Translational Research Fellow at The Christie and the clinical contact for patients on the study, explained: "Patients respond differently to immunotherapy (搜索), both in the benefit they receive and in the side effects they experience. By following patients throughout treatment and studying how their immune system changes over time, the MANIFEST platform aims to better understand the biology of response, resistance and toxicity."
Toward Predictive Biomarkers and New Targets
The ultimate translational goal of MANIFEST is to identify predictive biomarkers and novel therapeutic targets that could unlock a next generation of more precise, effective, and better-tolerated immunotherapies. Dr Miletić added: "We hope this will be one of the keys to discovering predictive biomarkers and new therapeutic targets, helping to unlock the next generation of more precise, effective and better-tolerated immunotherapies."
The study is supported by The Christie Charity, which has invested in clinical research fellowships to operationalize the ambitious protocol. Dr Miletić is working closely with consultant colleagues across multiple disease sites, Translational Research Programme Lead Dr Kate Brown, PhD, Senior Lead Research Nurse Suze Allibone and the Observational Research Studies team, as well as Jane Rogan and the MCRC Biobank team.
Patient-Driven Science
The significance of the patient contribution was underscored by the recent feature in The Observer UK, which highlighted the story of the study's first participant. Lim described this as "a powerful reminder that research is driven by patients whose generosity will benefit many others."
The MANIFEST study is made possible through collaboration between patients, clinicians, research teams, scientists, NHS partners, industry, and funders. Through deep clinical and biological profiling, the consortium is working to answer three fundamental questions: why cancer immunotherapies succeed, why toxicities occur, and how more precise, personalized treatment can be delivered.
