The development of PanChol (搜索) was motivated by the devastating 2010 cholera (搜索) outbreak in Haiti, which highlighted critical gaps in existing vaccination strategies. "The devastating cholera outbreak in Haiti in 2010 triggered my desire to create a new oral live-attenuated cholera vaccine," said co-senior author Matthew K. Waldor, MD, PhD, a principal investigator in the Division of Infectious Diseases in the Mass General Brigham (搜索) Department of Medicine.
Addressing Critical Global Health Need
Cholera (搜索) remains a major global health threat, with up to 4 million cases and 143,000 related deaths occurring annually. Approximately 1.3 billion people worldwide face risk of disease, which results from ingestion of Vibrio cholerae bacteria in contaminated water or food. These bacteria produce toxins that cause severe vomiting and diarrhea, with young children facing the highest mortality risk.
Current cholera (搜索) vaccines present significant limitations that PanChol (搜索) aims to address. "Existing vaccines made from inactivated bacteria require multiple doses and are not that efficacious, particularly in young children, the population most at risk of dying from cholera," Waldor explained. "Our research on the cholera pathogen suggested ways to make improved vaccines."
Innovative Vaccine Design and Development
The research team derived PanChol (搜索) from a weakened version of the Vibrio cholerae strain currently responsible for most global cholera (搜索) cases. The scientists incorporated specific characteristics to ensure the live vaccine's safety and prevent it from regaining the ability to produce toxins and cause disease.
This approach represents a departure from traditional inactivated bacterial vaccines, potentially offering enhanced protection through a single oral dose rather than the multiple doses required by existing options.
Phase 1 Trial Results
The 57-participant phase 1 trial employed a dose-escalation design to identify the most effective dosage. Initially, participants received increasing doses of PanChol (搜索) to establish optimal dosing parameters. Subsequently, additional participants were randomly assigned to receive either one oral dose of PanChol or placebo.
The trial achieved its primary safety objectives, with researchers observing that adverse events were typically mild and transient. Whole-genome sequencing of PanChol (搜索) isolated from vaccinated participants' stool samples confirmed the vaccine's genomic stability, a critical safety requirement.
PanChol (搜索) shedding, which serves as a marker for vaccine replication in the intestine, was detectable across all vaccine doses tested. Most significantly, 100% of vaccinated participants mounted strong immune responses against Vibrio cholerae antigens within two weeks of vaccination.
Clinical Significance and Future Development
"This work represents a significant step forward in our efforts to combat a devastating disease and shows what can be accomplished at MGB given the depth of our scientific talent," said co-senior author Lindsey Baden, MD, vice president of clinical research at Mass General Brigham (搜索).
The successful phase 1 results position PanChol (搜索) for testing in endemic regions where cholera (搜索) poses the greatest threat. An additional phase 1 trial is planned to begin in Lusaka, Zambia in early 2026, representing a crucial step toward evaluating the vaccine's performance in populations most at risk.
Global Impact Potential
The development team envisions deploying PanChol (搜索) in areas where it could have the greatest life-saving impact. "We hope our vaccine will be deployed where it is most needed to save lives—in places where cholera (搜索) is endemic and during outbreaks," Waldor stated.
The single-dose oral formulation could provide significant logistical advantages in resource-limited settings and emergency outbreak responses, potentially improving vaccination coverage and reducing the burden of cholera (搜索)-related morbidity and mortality worldwide.
