Samples collected on July 7, 8, 9, and 14 from the John M. Asplund Wastewater Treatment Facility tested positive for measles (搜索) virus RNA. The facility serves most of Anchorage's residents, including Joint Base Elmendorf-Richardson, though it does not cover Eagle River or Girdwood.
"These findings really tell us that at least one person, but maybe several people with measles (搜索) were in the Anchorage sewer shed catchment area between around July 7th through the 14th," said Dr. Joe McLaughlin, chief of the Alaska section of epidemiology and state epidemiologist. "It doesn't tell us who they were. It doesn't tell us where exactly they were in the sewer shed, and it also doesn't tell us whether transmission is occurring."
Vaccine-Strain Differentiation Confirms Wild-Type Virus
The wastewater testing methodology employed in Anchorage can distinguish between wild-type measles (搜索) virus and the vaccine-attenuated strain. Clayton Weingartner, program manager for healthcare acquired infections and antimicrobial resistance at the Division of Public Health and manager of the wastewater monitoring program, confirmed that the detected RNA was from the wild-type strain — the form carried by infectious individuals, not someone recently immunized.
"This testing does differentiate between wild-type measles (搜索), which an infectious person would carry, or the vaccine attenuated version, which somebody who was recently immunized, you know, could end up inputting into the sewer shed," Weingartner explained.
Anchorage has participated in a wastewater scanning program since 2024, monitoring for flu A and B, H5N1 bird flu, RSV, and SARS-CoV-2 in addition to measles (搜索). The Asplund plant is the only Alaska site participating in WastewaterSCAN, a privately operated advanced testing program. Weingartner noted that the measles detection process is complex: "It's an expensive test, and it takes longer to get the results," which accounts for the delay in receiving data from more recent sampling.
Low Vaccination Rates Heighten Vulnerability
Health officials expressed particular concern about Anchorage's vaccination coverage. Kristen VanderEnde, epidemiologist with the Anchorage Health Department (搜索), highlighted the gap between current rates and the threshold required for community protection.
"To prevent a measles (搜索) outbreak, you need coverage with two doses of measles vaccine of 95%. Most recent data from Anchorage show that we are under 80%. So we're vulnerable," VanderEnde said.
The MMR vaccine provides approximately 97% efficacy against measles (搜索), according to Dr. McLaughlin. "Fortunately, what we know about the MMR vaccine is it's one of the most effective vaccines that we have in the United States," he said. "The measles component alone confers about 97% efficacy against protecting against measles, which is remarkably high."
National Context and Seasonal Risk
The wastewater detection comes amid a significant national measles (搜索) resurgence. The Centers for Disease Control reports 2,318 confirmed measles cases in the U.S. as of July 23, 2026 — a figure that already exceeds all measles cases reported nationwide in 2025. Dr. McLaughlin noted there are more measles cases in the United States now than at any time in the past 35 years.
Alaska has recorded only one confirmed case in 2026, reported in February, though that individual was tested and diagnosed on the East Coast and was no longer infectious upon returning to Alaska. The last confirmed case for an Anchorage resident occurred in May 2025, with a visitor to the municipality reporting a case in October 2025.
Peak tourist season compounds the risk. "We're at peak tourist season right now, so there are people coming into Alaska from the Lower 48 and from many different countries, and they're bringing with them, some of them, different pathogens," McLaughlin said. "And one of them can be measles (搜索). And we know that measles is the most contagious virus that we're aware of on the planet."
Early Warning Function of Wastewater Surveillance
Wastewater monitoring has demonstrated utility as a sentinel system for measles (搜索). VanderEnde noted that "communities in the lower 48, there have been examples where they have seen a detection in wastewater in the weeks and days before seeing a measles case."
Symptoms of measles (搜索) typically appear one to two weeks after infection and include high fever, runny nose, red eyes, and a skin rash. The disease can be severe and potentially fatal, particularly in young children. The CDC reported three measles deaths in the United States in 2025.
Health officials urge anyone with suspected measles (搜索) to call their healthcare provider before visiting a clinical setting. "Measles is incredibly infectious," VanderEnde said. "If someone were to just walk into an emergency room or a pediatrician's office, they could unknowingly be exposing many more people."
With the school year approaching, both state and municipal authorities are concerned about potential transmission in educational settings. "This can really spread in these schools. In some places, it's caused disruptions and school closures," VanderEnde warned.
