Mesenchymal Stem Cell Therapy Shows Promise for Rare "Butterfly Skin" Disease in Spanish Clinical Trial

The Phase I/II study, conducted by researchers from multiple Spanish institutions, showed that intravenous infusions of stem cells from family donors reduced itching, sleep disturbances, and fatigue in pediatric patients.
The MesenSistem-EB trial enrolled eight pediatric patients who completed the full treatment protocol of three mesenchymal stem cell infusions. According to the research team, all patients showed overall reduction in pruritus, improved sleep quality, and lower levels of fatigue following treatment.
Novel Immunomodulatory Approach
Unlike previous therapeutic strategies that focused primarily on replenishing collagen 7 (搜索), this study targeted the patient's immune system. "The infused mesenchymal stem cells (搜索) act as regulators of the immune system, helping to control the state of permanent inflammation that deteriorates the health and well-being of patients," explained Dr. María José Escámez, the study's lead investigator from CIBERER (搜索)-ISCIII and IIS-FJD (搜索).
The researchers used bone marrow-derived mesenchymal stem cells (搜索) from family (haploidentical) donors. These cells possess high immunomodulatory capacity and the ability to secrete repair factors, making them particularly suitable for treating inflammatory and degenerative diseases.
Safety Profile and Biomarker Discovery
The trial confirmed the safety of this treatment approach, with no serious adverse events associated with the infusions during the one-year follow-up period. The research team successfully stabilized systemic inflammation indicators, including CRP (搜索) and fibrinogen levels, ensuring that patients' conditions did not worsen throughout the study period.
A significant breakthrough from the study was the identification of two predictive biomarkers: MCP1 (搜索) and sCD40L (搜索). Blood levels of these molecules can predict which patients will respond best to the treatment. "This represents an important step toward personalized medicine in rare diseases," noted Marcela Del Río Nechaevsky, a researcher at UC3M (搜索) Department of Bioengineering and IIS-FJD (搜索).
Understanding Butterfly Skin Disease
RDEB (搜索) is characterized by extreme fragility of the skin and mucous membranes, causing blisters and wounds at the slightest touch. The condition is associated with a systemic chronic inflammatory response that significantly impairs quality of life and affects life expectancy. According to the research team, the disease currently affects approximately 500,000 people worldwide.
Collaborative Research Effort
The study was conducted by researchers from Universidad Carlos III de Madrid (搜索) (UC3M (搜索)), Centro de Investigaciones Energéticas, Medioambientales y Tecnológicas (CIEMAT (搜索)), Fundación Jiménez Díaz Health Research Institute (搜索) (IIS-FJD (搜索)), CIBER Rare Diseases (CIBERER (搜索)), and La Paz University Hospital (搜索) in Madrid. Additional participation came from Complutense University of Madrid and Gregorio Marañón General University Hospital, in collaboration with Great Ormond Street Hospital in London.
The research received funding from the Institute of Health Carlos III (ISCIII), the Spanish State Research Agency (AEI), and patient associations DEBRA-Spain (搜索) and Berritxuak (搜索). The patient associations played a particularly important role, co-funding and supporting the clinical trial from its inception.
"For families, seeing science advance in the control of pain, itching, and sleep quality is a beacon of hope. This treatment doesn't just treat the skin; it treats the daily lives of children," stated the patient associations.
Future Therapeutic Implications
The researchers conclude that this therapeutic approach could become a fundamental complementary strategy to mitigate inflammatory escalation during critical phases of the disease. "The MesenSistem-EB clinical trial lays the groundwork for future combined protocols that could transform the prognosis of butterfly skin (搜索) internationally," indicated Lucía Martínez Santamaría, another study author from UC3M (搜索) Department of Bioengineering and IIS-FJD (搜索).
The study results were recently published in the journal Frontiers in Immunology, marking an important advancement in the treatment of this devastating rare disease.
