Methylphenidate May Reduce Psychosis Risk in Children with ADHD, Large Finnish Study Shows

A comprehensive analysis of nearly 4,000 children and adolescents with ADHD (搜索) in Finland has revealed that methylphenidate, the most commonly prescribed ADHD medication, does not increase the risk of psychosis (搜索) and may actually provide protective effects when used long-term in younger patients. The findings, published Wednesday in JAMA Psychiatry, challenge longstanding concerns about stimulant medications potentially triggering psychotic episodes in vulnerable children.
Study Findings Challenge Previous Concerns
Researchers from the University of Edinburgh and University College Dublin analyzed health records of Finnish children diagnosed with ADHD (搜索), finding no difference in psychosis (搜索) risk between those treated with methylphenidate—including brand names Ritalin and Concerta—and those who were not. More significantly, sustained methylphenidate treatment prescribed to children under age 13 for three to four years appeared to lower the risk of developing psychotic conditions.
"We found overall that although we know that this group is at an increased risk of psychosis (搜索), it wasn't to do with the medication," said Dr. Ian Kelleher, chair of child and adolescent psychiatry at the University of Edinburgh and the senior author of the study. "That risk would seem to be due to other factors."
The research addresses a critical clinical concern, as children with attention-deficit/hyperactivity disorder have been identified to be at greater risk of developing psychotic conditions such as bipolar disorder (搜索) or schizophrenia (搜索) compared with neurotypical children. Some previous studies had suggested a potential link to stimulants prescribed for ADHD (搜索).
Genetic Factors May Drive Psychosis Risk
Kelleher suggested that the link between ADHD (搜索) and psychosis (搜索) may be due to shared genetic risk factors between the disorders and could be completely independent of medication. This finding has significant implications for the approximately 3.5 million children between the ages of 3 and 17 in the United States who are prescribed ADHD medications.
Dr. Christian Kohler, professor of psychiatry at the hospital of the University of Pennsylvania, described the results as "thought-provoking and not expected." However, he noted that the findings don't completely rule out a possible association in older age groups, particularly people in later adolescence or early adulthood who have been newly diagnosed with ADHD (搜索).
Age-Related Considerations and Dopamine Effects
Kohler explained that one of the concerns with stimulants is that they increase the availability of dopamine, a key brain chemical, in the limbic system—a part of the brain linked to emotional regulation. This mechanism may propel some vulnerable individuals to develop erratic thinking and hallucinations.
"What if you're diagnosed with ADHD (搜索) at 16 and 17 and you're being put on a stimulant?" Kohler said. "As clinicians we see, not infrequently, people experiencing some cognitive dysfunction later in adolescence, being diagnosed with ADHD, put on a stimulant and then they experience psychosis (搜索) within a couple of months."
Kohler suspects there might still be a tipping point, perhaps starting in the late teens, where the risk of psychosis (搜索) does increase following stimulant treatment.
Amphetamine Medications Require Further Study
The study's focus on methylphenidate leaves questions about amphetamine-based medications like Adderall, which are more commonly prescribed to adults with ADHD (搜索) and privately insured patients. Research has shown that patients prescribed amphetamines are more likely to develop psychosis (搜索) than those who receive methylphenidate.
Dr. Jeffrey Newcorn, director of the division of ADHD (搜索) and learning disorders at the Icahn School of Medicine at Mount Sinai, noted that psychosis (搜索) risk is thought to be higher in patients prescribed amphetamines. "This study doesn't say you can't have amphetamine psychosis," he said. "It's very rare with the doses we give, if people take them properly, but it very, very occasionally happens."
A 2024 study found that people who take high doses of amphetamines, including Vyvanse and generic versions, for ADHD (搜索) had more than five times greater risk of developing psychosis (搜索) or mania.
Because amphetamines are less commonly prescribed in Europe, there were too few children in the Finnish dataset to draw accurate conclusions about drugs such as Adderall and psychotic disorders, Kelleher said.
Potential Neuroprotective Mechanisms
Kohler was particularly intrigued by the study's finding that methylphenidate can play a preventive role in younger children. This finding aligns with prior research conducted in laboratory mice and rats, which found that long-term methylphenidate treatment in juvenile animals normalizes some abnormalities in dopamine-rich brain regions.
"Does it maybe balance or repair an early deficit [in the brain], so that it doesn't become a risk for developing psychosis (搜索) later on," Kohler said.
Clinical Implications and Reassurance
The researchers emphasized that medication dose, timing of treatment, and the stability of the patient's condition may combine to play a critical role in whether stimulants are protective or liable to cause further problems.
"It's reassuring that the risk of a serious lifelong psychotic disorder is not increased by [this] medication," Newcorn said, noting that the research should provide relief for parents of children with ADHD (搜索).
While many questions remain to be answered, particularly regarding amphetamine-based medications and treatment timing, the study provides important evidence that methylphenidate treatment in younger children with ADHD (搜索) does not increase psychosis (搜索) risk and may offer protective benefits when used appropriately.
