mRNA Acne Vaccine Enters Early Clinical Trials as Sanofi and Armora Biosciences Pursue Preventive Approaches

A new generation of vaccines targeting acne (搜索) is advancing into human trials, marking a potential paradigm shift in how one of the world's most common skin conditions could be treated. Rather than managing breakouts with daily creams, antibiotics, or oral medications, researchers are pursuing shots that could prevent acne by modulating the immune system's response to bacteria naturally residing on the skin.
At least two companies with Massachusetts ties are developing novel vaccines, among the first of their kind, that target Cutibacterium acnes (搜索) (C. acnes), the bacterium associated with inflammatory acne (搜索) lesions.
Sanofi's mRNA Vaccine Enters the Clinic
Sanofi, the French pharmaceutical giant with offices and labs in Cambridge, Massachusetts, has advanced its mRNA-based acne (搜索) vaccine into early Phase 1 and Phase 2 clinical trials. The studies are enrolling participants across multiple countries, including the United States, Canada, Australia, and Singapore.
The vaccine leverages the same mRNA technology platform used in Pfizer's and Moderna's COVID-19 shots, but instead of targeting a virus, it trains the immune system to attack and eliminate acne (搜索)-causing bacteria found deep in the skin. The shot is also designed to target "nasty components" released by the bacteria that can trigger inflammation, according to Jean-Francois Toussaint, global head of vaccine research and development at Sanofi.
"We basically remove the bad guys" and leave beneficial bacteria unharmed, Toussaint said, noting that the vaccine is not expected to change the composition of microbes on the surface of the skin, which help the body function properly.
The company is enrolling approximately 800 participants in a clinical trial testing the vaccine in individuals with moderate to severe acne (搜索). Participants must be healthy adults between 18 and 45 years old and will receive up to three shots. A smaller group with mild acne is also being studied. Initial results from the trial are expected in 2028, Toussaint said.
While clear skin is the ultimate goal, Toussaint acknowledged that "it would be a big win" if the vaccine could help those with more serious acne (搜索) improve to milder cases.
A Different Approach: Targeting the HylA (搜索) Enzyme
Researchers at the University of California San Diego School of Medicine, led by Dr. George Liu, professor and head of pediatric infectious diseases, have pursued a distinct strategy. Rather than eliminating the bacteria themselves, Liu's team targeted a specific enzyme called HylA (搜索) produced by acne (搜索)-associated strains of C. acnes.
When released by the bacteria, HylA (搜索) breaks down hyaluronic acid — a molecule that helps skin retain elasticity — into large fragments. These fragments activate the immune system and can provoke the inflammatory response that causes acne (搜索). A vaccine, Liu explained, could create antibodies that block the enzyme from cutting up hyaluronic acid.
In mouse models, stopping the enzyme from interacting with hyaluronic acid prevented acne (搜索), according to results published in 2023 in Nature Communications.
"The vaccine targets this pro-inflammatory enzyme and not the bacteria," Liu said. This approach aims to reduce inflammation while preserving the normal bacteria that live on healthy skin.
Liu's research has since been licensed by Armora Biosciences (搜索), a company based in Boston and New York focused on immunodermatology. Tomasz Zastawny, the company's CEO and co-founder, described the strategy as avoiding "carpet bombing with antibiotics or other therapies, which gives you several types of side effects."
The Clinical Need
Acne (搜索) affects over 40 million Americans and is the number one reason patients visit a dermatologist, according to Dr. Joshua Zeichner, an associate professor of dermatology at Mount Sinai Hospital in New York. Dermatologists estimate that approximately 85% of adolescents experience acne at some point, while adult acne — defined as acne that continues or appears after age 25 — is increasingly common.
"Acne (搜索) is not an infection," Zeichner said. "It's an inflammatory response" linked to the immune system "overreacting to bacteria that are part of your skin's natural microbiome."
The condition can inflict lasting damage, including physical scars, anxiety, and depression. "There is a very significant medical need linked to acne (搜索)," Toussaint said. "Some people may see it like just a cosmetic disease … I truly believe that acne is a skin, but also a mental health issue."
Current treatments — including topical creams, antibiotics, and oral medications such as isotretinoin (widely sold as Accutane) — largely manage the condition rather than cure it. Long-term antibiotic use can contribute to resistance and disrupt the body's microbiome. Isotretinoin, while highly effective, can cause serious side effects and requires regular monitoring. Some estimates suggest that a person with acne (搜索) in the United States might spend an average of $150 to $200 each year on over-the-counter products, while prescription medications like isotretinoin can cost more than $1,000 for a standard course depending on insurance coverage.
"To date, there is still no safe and effective treatment that can prevent and cure acne (搜索) entirely," Sanofi said in a statement explaining the rationale behind the vaccine research.
Cautious Optimism and Challenges Ahead
Despite the excitement, experts caution that significant hurdles remain. "In the case of the skin, it's very difficult to form your own immunity," said Dr. Abigail Waldman, a dermatologist at Mass General Brigham. "That means it's going to be difficult for them to get a very focused, effective vaccine."
Because acne (搜索) has more than one trigger — including the amount and quality of sebum, an oily substance produced by the skin — a vaccine targeting only bacteria may not be effective for everyone.
Researchers are also mindful of potential long-term impacts. "The onus is on us, people who do this, to carefully design any vaccine that we put in humans," Liu said, acknowledging that altering the skin microbiome could have unforeseen consequences.
Early-stage trials primarily focus on safety and immune response. If results are promising, the vaccine will still need to undergo larger Phase 2 and Phase 3 clinical trials involving thousands of participants before regulatory approval — a process that can take many years. A long-term extension study is planned to assess how durable the effect may be.
Broader Implications
If successful, Sanofi's shot could be one of the first of its kind to prevent inflammation linked to bacteria. "Scientifically, it could open doors to other vaccines for other inflammatory diseases," Toussaint said.
Ross Kedl, a professor in the department of immunology and microbiology at the University of Colorado Anschutz School of Medicine, described the research as moving in an "exciting direction," calling it "a really good example of using the more subtle dials that we have in the immune system to control inflammatory overexuberance."
Kedl also noted that an acne (搜索) vaccine could have implications beyond dermatology: "Anytime you make a vaccine that provides benefit to people concretely, that you can visually see, that tends to be good for the world of vaccines at large."
