Multi-Strain Probiotic Shows Promise in Reducing Bacterial Vaginosis Recurrence

A groundbreaking clinical trial has demonstrated that a multi-strain probiotic (搜索) therapy can significantly reduce bacterial vaginosis (搜索) (BV (搜索)) recurrence by restoring protective bacteria to the vaginal microbiome. The phase 1 randomized trial, conducted across sites in the United States and South Africa, found that just one week of treatment was sufficient to establish beneficial bacteria that persisted for months after therapy ended.
Novel Multi-Strain Approach Shows Superior Results
The VIBRANT (Vaginal lIve Biotherapeutic RANdomized Trial) enrolled 90 participants from Vulindlela, South Africa and Boston, US. Each participant received antibiotics followed by one of three treatments—placebo tablets, 6-strain tablets, or 15-strain tablets containing multiple strains of Lactobacillus crispatus—for seven days.
Results showed that two-thirds of participants (66%) developed protective L. crispatus bacteria within the first five weeks. Remarkably, nearly half of that group still maintained the beneficial bacteria at 12 weeks, even though some participants received only three days of active treatment. Participants with protective bacteria at five weeks were significantly less likely to experience BV (搜索) recurrence during the study timeframe.
"For decades, we've relied on medications that clear the infection but do not restore beneficial bacteria, leaving the vaginal environment vulnerable," said corresponding author Caroline Mitchell, MD, MPH, director of the Vulvovaginal Disorders Program at Massachusetts General Hospital. "We wanted to see if we could 're-seed' that environment with protective bacteria and help the body stay healthy on its own."
Understanding Treatment Response Mechanisms
Parallel research from the Kwon Lab at the Ragon Institute (搜索), published in Cell Host & Microbe, provided crucial insights into why some women respond better to bacterial therapy than others. The study analyzed over 1,100 samples from 213 trial participants using microbiome sequencing, L. crispatus strain analysis, immune profiling, and clinical data.
The research revealed that 12 weeks after starting treatment with LACTIN-V (a single-strain live biotherapeutic), 30% of women developed a healthy, L. crispatus-dominant vaginal microbiome, compared to just 9% of women receiving placebo. This colonization was primarily driven by the bacterial strain in the treatment, though in some cases women's own naturally occurring L. crispatus took over.
Critically, the study identified that a woman's pre-treatment vaginal microbiome predicted treatment response. Women whose microbiomes were predominated by Prevotella (搜索) or Gardnerella (搜索) species saw the greatest benefit, while those predominated by Candidatus Lachnocurva vaginae (搜索) (BVAB1), a bacterium strongly associated with BV (搜索), showed little or no improvement.
Addressing a Global Health Challenge
Bacterial vaginosis (搜索) affects approximately 30% of women globally and more than 25% of reproductive-age women, with disproportionate impact on women of lower socioeconomic status and members of racial and ethnic minority groups. The condition causes discharge, odor, and irritation, and is associated with increased risk for preterm birth, HIV (搜索) acquisition, cervical cancer (搜索), and abnormal cell growth on the cervix.
Standard antibiotic treatment with metronidazole often clears BV (搜索) initially, but the condition recurs in over half of women within a year. Up to 60% of women experience BV recurrence within six months of antibiotic treatment, largely because the vaginal microbiome fails to convert to a bacterial community dominated by beneficial L. crispatus.
"HIV (搜索) remains a major challenge in young women in Africa and reducing their risk of infection has been an elusive goal," said Disebo Potloane, MB, ChB, who led the trial at CAPRISA (搜索)'s rural South African research clinic. "The encouraging results of the VIBRANT trial bode well for development of strategies aimed at removing a fundamental cause of their HIV risk."
Clinical Implications and Future Directions
The research team identified several factors linked to successful L. crispatus colonization, including lower bacterial levels and vaginal pH after metronidazole treatment, as well as certain pre-treatment immune profiles. These findings offer a roadmap for developing more precise and effective treatments.
"This study's results offer key insights for vaginal health," says Seth Bloom, co-first author of the Kwon Lab study. "Our results offer a roadmap to guide development and use of LACTIN-V and similar live biotherapeutic products to improve the health of the millions of women worldwide who experience BV (搜索)."
The research team is planning follow-up trials to optimize the multi-strain treatment before initiating larger-scale clinical trials aimed at securing FDA approval of vaginal live biotherapeutics for BV (搜索). The studies represent a collaboration between investigators from Mass General Brigham (搜索), the Centre for the AIDS Programme of Research in South Africa (CAPRISA (搜索)), and the Vaginal Microbiome Research Consortium, with funding provided by the Gates Foundation (搜索).
