MUSC Launches Clinical Trial for Heart-Safe Triple-Negative Breast Cancer Treatment

The study, led by MUSC Breast Medical Oncologist Dr. Abirami Sivapiragasam, aims to provide effective treatment options for patients who cannot receive standard therapy due to cardiac complications.
Treatment Challenge for High-Risk Patients
Triple-negative breast cancer (搜索) patients typically receive a five-drug regimen combining immunotherapy and chemotherapy before surgery. The standard treatment successfully treats 65% of women in stage two or three of the cancer, according to Dr. Sivapiragasam. However, a major component of this regimen, Adriamycin, poses significant risks for patients with existing heart conditions.
"This is a drug that comes with a lot of side effects," Sivapiragasam explains. "Especially, a small minority of patients can have cardiomyopathy (搜索), which means weakening function of the heart muscles."
This limitation forces physicians to develop individualized approaches for patients who cannot tolerate the full standard regimen. "It makes me feel that, 'why don't we have a uniform approach to treat these patients?', but the downside to that is that this is a hard disease or this is a hard population to study, and many clinical trials that you look at don't include patients with this disease," Sivapiragasam notes.
Novel Three-Drug Protocol
The clinical trial will test a modified treatment approach using three of the five drugs from the standard regimen, eliminating the cardiotoxic components while maintaining therapeutic efficacy. The study plans to enroll 43 patients over two years, with participants receiving the three-drug combination on a weekly basis.
The treatment protocol includes a structured assessment timeline. "After 12 weeks, we're going to assess the response with an MRI," Sivapiragasam explains. "If somebody has a complete response with the 12-week regimen, then we are comfortable sending those patients to surgery. Whereas if they don't have a complete response, then we plan to give them six more weeks of the treatment and then get them to surgery."
Early Success Story
The approach has already demonstrated promising results in clinical practice. Georgetown resident Julia Holmes, diagnosed with triple-negative breast cancer (搜索) in December 2023, successfully completed the modified treatment protocol. "I just kept feeling under there, and I felt a lump," Holmes recalls. "So, I went to the doctor that same day and found out what was going on."
Holmes received the three-drug treatment under Sivapiragasam's care and achieved a complete response within 12 weeks, completing her treatment with radiation therapy in June 2024. She is now cancer-free, representing one of several patients who have succeeded with this modified approach.
Future Implications
The complete trial is expected to take approximately eight years to finish. If successful, Sivapiragasam hopes to expand the study to the national level, potentially establishing a new standard of care for triple-negative breast cancer (搜索) patients with cardiac comorbidities.
This research addresses a significant unmet medical need by providing a treatment option for a patient population that has been largely excluded from clinical trials due to their complex medical conditions. The study represents an important step toward developing more inclusive and personalized cancer treatment approaches.
