New Dark Borosilicate Contrast Agent Shows Promise for Enhanced CT Imaging of Bowel Pathologies

A collaborative pilot study involving researchers from the University of Washington School of Medicine (搜索), Mayo Clinic, University of California San Francisco, and Nextrast Inc. has demonstrated that a novel dark borosilicate oral contrast agent significantly improves CT imaging visualization of bowel (搜索) structures and disease compared to conventional contrast agents.
The feasibility study, which examined 32 patients, compared CT scans using the new dark borosilicate agent against traditional water- and iodine-based oral contrast agents to assess anatomical structures and disease presence. Radiologists identified clinically relevant findings in seven patients that were not detected using standard contrast agents.
Enhanced Detection Capabilities
"The novel dark borosilicate contrast agent (搜索) has the potential to improve the assessment of numerous pathologies, including the detection of cancer (搜索) and inflammatory disorders," said Dr. Achille Mileto, a radiologist at the University of Washington School of Medicine (搜索) and the study's first author.
The new agent, which received FDA clearance in late 2023, is specifically designed to enhance image clarity in the stomach (搜索) and small bowel (搜索). According to Dr. Joel Fletcher, lead author on the study and medical director of the Mayo Clinic CT Clinical Innovation Center, the agent addresses common challenges in diagnosing bowel (搜索) diseases by helping detect pathologies within and outside of the bowel wall in ways current imaging agents cannot.
Targeting Diagnostic Shortcomings
CT scans performed with the new agent demonstrated superior performance in displaying pathologies in the stomach (搜索) and proximal small bowel (搜索), areas where CT with traditional oral contrast performs poorly. "Pathologies within the lumen will be better displayed because the lumen is more distended, and the brightness of the pathologies on the CT images will be increased, making them more conspicuous," Fletcher explained.
Dr. Benjamin Yeh, a study coauthor and radiologist at the University of California at San Francisco who is also a cofounder of Nextrast, emphasized the agent's design advantages. "We carefully designed this new class of oral contrast agent to overcome many of the diagnostic shortcomings of existing contrast agents for a broad range of diseases," he said. "The clinical Phase 2 results confirm that our dark agent can reveal previously impossible-to-detect findings, including very small bowel (搜索) tumors and subtle inflammatory conditions (搜索) that are often missed with conventional agents at CT."
Clinical Implications
The findings suggest this new agent significantly improves visualization of inflammatory bowel disease (搜索) (IBD) or cancer (搜索) within the abdomen, and may provide earlier and more confident diagnoses. The agent is ingested by patients prior to CT imaging and works by enhancing the contrast between bowel (搜索) structures and surrounding tissues.
The study team, which included radiologists and imaging scientists from all four participating organizations, acknowledged that larger trials are needed to validate the findings. However, the initial results show promise for improving diagnostic capabilities in gastrointestinal imaging, particularly for conditions that are challenging to detect with current contrast agents.
