The Chronic Venous Thrombosis: Relief with Adjunctive Catheter-Directed Therapy (C-TRACT) Trial, sponsored by the National Institutes of Health and co-led by researchers at Washington University School of Medicine in St. Louis, enrolled 225 patients across 29 U.S. sites. Results published April 13 in the New England Journal of Medicine show that stent placement reduced severe post-thrombotic syndrome (搜索) cases from 93% to just 40% of patients six months after treatment.
Addressing a Significant Unmet Medical Need
More than 300,000 Americans are diagnosed with deep vein thrombosis (搜索) annually, with the condition contributing to as many as 100,000 deaths each year when combined with pulmonary embolism (搜索). Post-thrombotic syndrome (搜索) develops in approximately 40% of survivors, causing leg pain, swelling, skin discoloration, impaired mobility and reduced quality of life.
"We see a lot of patients who have moderate or severe post-thrombotic syndrome (搜索) and have a tough time conducting their daily activities and maintaining a good quality of life. Many patients are unable to walk without pain or work productively," said lead author Suresh Vedantham, MD, an interventional radiologist and professor of radiology in Mallinckrodt Institute of Radiology at WashU Medicine.
Trial Design and Patient Population
The randomized controlled trial included patients who had been treated for deep vein thrombosis (搜索) for at least three months and had blockage of the iliac vein (搜索) causing substantial disability. Participants were randomly assigned to receive either a stent plus standard treatment with compression and blood-thinning medications, or standard treatment alone.
The stent procedure involves placing a mesh tube into the affected vein to reinforce and expand it, restoring blood flow that had been restricted by scar tissue formation following the initial blood clot. The operation takes two to three hours and requires only tiny incisions.
Significant Clinical Improvements
At baseline, 93% of study participants met clinical criteria for severe post-thrombotic syndrome (搜索). Six months post-treatment, the stent group showed marked improvement, with only 40% still experiencing severe symptoms compared to 61% in the standard care group.
Beyond reducing syndrome severity, patients who received stents reported improved venous symptoms and overall quality of life. On a 100-point quality of life scale, the stent group scored 14 points higher than the standard care group, representing what researchers characterized as a large improvement.
Breaking New Ground in Treatment Options
"The condition has been undertreated, in part because there have not been evidence-based options available. This study is the first large randomized trial to show that this is a treatable condition, giving patients meaningful relief from this disease," Vedantham explained.
Traditional treatment approaches focus primarily on preventing new clot formation through blood-thinning medications and managing symptoms through compression therapy. However, these methods do not address the underlying vascular damage that causes post-thrombotic syndrome (搜索).
"Post-thrombotic syndrome (搜索) can be life-altering, yet we've had very few effective treatment options," said senior author Sameer Parpia, PhD, data coordinating center principal investigator and associate professor in the departments of Oncology and Health Research Methods, Evidence, & Impact at McMaster University. "This trial provides clear evidence that targeted therapy can make a meaningful difference for patients living with this condition."
Clinical Impact and Future Directions
The trial results establish a new treatment paradigm for post-thrombotic syndrome (搜索), potentially benefiting the estimated 120,000 Americans who develop this condition annually following deep vein thrombosis (搜索). The procedure directly addresses the underlying cause of symptoms by restoring blood flow through damaged veins.
Vedantham noted that the trial will provide a foundation for improving patient care for post-thrombotic syndrome (搜索) and help physicians better understand treatment options they can recommend for patients seeking relief from this condition.
The study was supported by grants from the National Heart, Lung, and Blood Institute, with additional backing from the Institute for Clinical and Translational Sciences at Washington University in St. Louis and the Canadian Institutes of Health Research.
