Novel Radiotracer Shows Promise for Non-Invasive Endometriosis Diagnosis in Phase 2 Trial

The study, published in The Lancet Obstetrics, Gynaecology, & Women's Health, evaluated 99mTc-maraciclatide in 19 women scheduled for laparoscopic surgery.
Breakthrough in Detecting Early-Stage Disease
The DETECT study revealed that 99mTc-maraciclatide successfully identified endometriosis (搜索) in 16 out of 19 cases when compared to surgical findings, with no false positives reported. Most significantly, the radiotracer detected superficial peritoneal endometriosis (搜索) (SPE), the earliest and most common form of the condition that affects 80% of all laparoscopically diagnosed cases but cannot be reliably detected by conventional imaging techniques.
"Superficial peritoneal endometriosis (搜索), the most prevalent subtype of endometriosis (搜索), currently evades reliable detection, leaving women no choice for diagnosis other than invasive surgery," said Professor Krina Zondervan, Head of Department at the Nuffield Department of Women's and Reproductive Health (搜索) at the University of Oxford and co-lead on the study.
Targeting Angiogenesis for Enhanced Detection
The radiotracer works by binding to αvβ3 integrin (搜索), which is upregulated during angiogenesis—the formation of new blood vessels that represents a cardinal feature of endometriosis (搜索). Patients received an intravenous infusion of the tracer before undergoing SPECT-CT imaging, which successfully visualized endometriotic lesions across all disease subtypes.
Of particular clinical significance, ten participants with SPE had undergone traditional imaging with transvaginal ultrasound and/or MRI within the previous 12 months, none of which had detected the condition. The study also successfully identified two cases of thoracic endometriosis (搜索), demonstrating the tracer's potential applicability across various patient populations.
Clinical Impact and Current Diagnostic Challenges
Endometriosis (搜索) affects approximately one in ten women and can significantly impact fertility and quality of life. The condition occurs when tissue similar to the womb lining grows elsewhere in the body, causing symptoms including painful periods, painful bowel movements, pain during urination, and pain during or after sexual intercourse.
Research by Endometriosis (搜索) UK indicates that diagnostic delays extend to 11 years for women from ethnic minority communities. Health Secretary Wes Streeting recently highlighted this problem in the government's renewed Women's Health Strategy, noting that endometriosis can progress during these delays, leading to more severe symptoms and limiting fertility options.

Beyond diagnosis, the imaging agent showed potential as a monitoring tool for disease recurrence and treatment response. "These exciting findings indicate that maraciclatide (搜索) offers a highly promising diagnostic and monitoring tool, particularly for superficial peritoneal endometriosis (搜索), which is the most common and yet the hardest type of endometriosis (搜索) to identify," said Dr. Tatjana Gibbons, lead author and investigator from the University of Oxford.
The development could significantly impact clinical research by providing a validated endpoint for new therapeutic development. Professor Christian Becker, Co-Director of the Endometriosis (搜索) CaRe Centre in Oxford, noted that if Phase 3 results confirm these findings, "maraciclatide (搜索) has the potential to be an extremely valuable tool, as it could both reduce diagnostic delays and provide a validated endpoint for the development of new therapeutics."
Path to Regulatory Approval
Serac Healthcare (搜索), the UK-based company developing maraciclatide (搜索), has received FDA Fast Track Designation and plans to initiate Phase 3 multi-centre international studies later this year. David Hail, CEO of Serac Healthcare, emphasized that the data from this representative patient population, including women receiving hormone therapy, provides evidence of the tracer's anticipated real-world performance.
The study was conducted as an exploratory, open-label, single-centre Phase 2 trial funded by the NIHR Oxford Biomedical Research Centre and Serac Healthcare (搜索), with scans performed at the Royal United Hospital in Bath. Patient acceptability was high, and the radiotracer was well tolerated throughout the study.
