A groundbreaking clinical trial is set to investigate a novel approach to protecting heart muscle during coronary procedures, potentially offering new hope for patients with unstable angina (搜索) requiring intervention. The study, launching January 2024 across two major medical centers in Jiangsu Province, China, will evaluate a comprehensive remote ischemic conditioning (搜索) (RIC) protocol delivered before, during, and after percutaneous coronary intervention (搜索) (PCI).
The double-blind, randomized controlled trial will enroll 574 patients with unstable angina (搜索) to receive either the RIC intervention or a sham procedure. The RIC protocol involves controlled cycles of temporary arm blood flow restriction using an automated cuff device, designed to trigger the body's natural protective mechanisms against cardiac injury.
"This study addresses a critical need in interventional cardiology," explains the research team. "Approximately 30% of PCI procedures result in some degree of perioperative myocardial injury (搜索), which can significantly impact patient outcomes."
The innovative three-phase conditioning approach represents a key advance in the field. Patients will receive three cycles of RIC before the procedure, one cycle during PCI, and three additional cycles post-procedure. Each cycle consists of three minutes of controlled upper limb ischemia followed by three minutes of reperfusion.
Beyond examining the primary endpoint of periprocedural myocardial injury (搜索), the study will track multiple secondary outcomes including:
- Major adverse cardiac events
- Arrhythmia incidence
- Coronary artery disease severity via Gensini scoring
- Cardiac function through echocardiography
- Quality of life measures using the Seattle Angina Questionnaire
The trial employs rigorous methodology, including stratified block randomization and comprehensive blinding of participants, healthcare providers, and outcomes assessors. Only the therapists delivering the intervention will know patient assignments.
"If successful, this protocol could transform how we protect the heart during coronary interventions," the research team notes. "The perioperative approach ensures patients receive protection during critical therapeutic windows, potentially offering more comprehensive cardiac protection than previous single-phase protocols."
The study will follow patients for 12 months to assess long-term outcomes. Results could have significant implications for improving the safety of PCI procedures and expanding treatment options for unstable angina (搜索) patients.
The trial is registered with the Chinese Clinical Trial Registry (ChiCTR2400079855) and has received full ethical approval. Findings are expected to be available by early 2026.
