Oak Park Fire Department Joins 30-Year Illinois Study Tracking Cancer Risk Through Biomarker Monitoring

The Oak Park Fire Department (搜索) has partnered with the University of Illinois Fire Service Institute (搜索) to participate in a long-term cancer risk study that will follow firefighters for up to 30 years, joining a statewide cohort that now includes more than 100 Illinois departments. The study, which launched in late 2024, aims to connect specific occupational exposures to health outcomes through direct biomarker measurement rather than relying on job title alone.
"This study brings together scientific expertise and resources that individual fire departments simply cannot replicate on their own," said JT Terry, Oak Park's fire chief. The department employs approximately 79 sworn firefighters, paramedics, and civilian personnel.
Study Design and Methodology
Participating firefighters provide blood and urine samples, complete a baseline health questionnaire and an annual cancer survey, and log their fire exposures after each incident. The study is designed to follow participants for up to 30 years, with enrollment open to firefighters aged 18 and older, including retirees, emergency medical responders, volunteers, and career personnel of any rank. Participation is voluntary and can be withdrawn at any time.
Farzaneh Masoud, director of research at the Illinois Fire Service Institute, explained the dual focus of the research: "We look at the biomarkers of exposure and biomarkers of effect, how those exposures can affect their body, their metabolism or their muscles." She added that through annual sample collections, researchers track "gene toxicity, how these conditions are affected due to the accumulation of toxicants and carcinogens in their bodies."
To overcome logistical barriers posed by firefighters' rotating shifts across dispersed stations, the institute operates a 40-foot mobile laboratory that travels directly to stations to collect samples.
The Scientific Context
The link between firefighting and cancer is well-established at the hazard level. In 2022, a working group of 25 experts from eight countries convened by the World Health Organization's International Agency for Research on Cancer classified occupational exposure as a firefighter as carcinogenic to humans—the agency's highest hazard category. The review found sufficient evidence in humans for mesothelioma (搜索) and bladder cancer (搜索), and limited evidence for colon, prostate, and testicular cancer (搜索), melanoma (搜索) of the skin, and non-Hodgkin lymphoma (搜索).
What remains unsettled, however, is which specific exposures on which kinds of fires drive which cancers, and how factors such as gear decontamination, fire station air quality, shift work, and diesel exhaust each contribute. Firefighters have historically been studied as a group defined by job title rather than by measured exposure, and women and volunteer firefighters have been underrepresented.
The Illinois Firefighter Cancer Risk Study is designed to close this gap by measuring exposure directly. The project focuses on how accumulated occupational toxicants—including per- and polyfluoroalkyl substances (PFAS (搜索)), polycyclic aromatic hydrocarbons (PAHs (搜索)), and heavy metals—relate to later cancer risk.
Benefits for Participating Firefighters
Terry framed the department's participation as serving both current members and future generations. "We hope our participation not only benefits our own members through enhanced screening and early detection but also contributes to research that will improve firefighter safety across Illinois and beyond," he said.
Masoud emphasized the broader mission: "They ultimately want to get ahead of cancer because it's all about early detection. So if we can understand cancer better, get ahead of it, there's going to be a lot of new knowledge that comes out of this research. We believe it will ultimately save lives, if not for this generation, then hopefully for the next."
What the Study Has Not Yet Done
It is important to note that this is an enrollment milestone, not a results announcement. The study has produced no findings to date. Cohort studies of this design can identify associations between measured exposures and later disease but cannot, on their own, prove causation. Early biomarker findings, when they emerge, will describe biological changes whose clinical significance may take years to interpret.
Firefighters outside Illinois can enroll in the CDC's National Firefighter Registry for Cancer, which has surpassed 40,000 members, making it the largest firefighter cohort in the country. The registry is open nationally to career and volunteer firefighters, active or retired, with or without a cancer diagnosis.
