Pediatric Long COVID Is Real: Landmark PECOS Study Maps Lasting Effects on Children

The Pediatric COVID Outcomes Study (PECOS), one of the largest and most comprehensive prospective studies of pediatric COVID-19 (搜索) to date, is beginning to answer a question that has lingered since the pandemic began: what does recovery look like for children months or years after infection? The collaboration between Children's National Hospital and the National Institutes of Health enrolled over 1,000 children and adolescents, following them over time and comparing those who had COVID-19 with a carefully screened group that had never been infected.
Unlike COVID outcomes studies that rely on retrospective reviews of medical records, PECOS uses detailed evaluations, physical exams, surveys, and in-person testing of participants. The study has produced a series of papers examining long-term symptoms, behavioral health, and cardiac outcomes, offering one of the clearest pictures yet of COVID-19 (搜索)'s lasting effects on children.
Long COVID in Children Is Real
The PECOS team identified a group of children who experienced persistent symptoms after COVID-19 (搜索) infection. Those symptoms varied by age and changed over time, but for some children they affected far more than physical health. Fatigue can make it difficult to get through a school day, headaches can interfere with concentration, and exercise intolerance can keep children from participating in sports and other activities central to childhood.
"There is a subset of kids who have symptoms which affect their life," said Roberta DeBiasi, MD, chief of pediatric infectious diseases at Children's National Hospital and co-principal investigator of the study. "Particularly when they're at a time of life when they should be attending school and being with their friends and developing socially and doing sports."
For families searching for answers, these symptoms are often difficult to explain and difficult to connect to a single diagnosis.
A Variable and Evolving Symptom Profile
Long COVID (搜索) does not look the same in every child. Researchers identified 20 symptoms that were more common among children who had recovered from their initial COVID-19 (搜索) infection than among those who were not infected. Among the most frequently reported were fatigue, headaches, gastrointestinal problems, persistent cough, difficulties with concentration or memory, muscle and joint pain, and trouble sleeping.
Symptoms seen in younger children — such as prolonged fever or dry persistent cough — often differed from those reported by adolescents and young adults. Some symptoms improved over time while others persisted or appeared later. This vast spectrum of symptom presentation is one reason diagnosing and treating these children can be challenging.
Dr. Uzma Hasan, Regional Director of Pediatric Infectious Diseases at Northern New Jersey Barnabas Health Medical Group, who helped open the first pediatric Long COVID (搜索) clinic in the country, noted that Long COVID is not limited to children who had severe infection. "Surprisingly, we see a lot of kids coming in who have either asymptomatic COVID infection or mild COVID symptoms that wind up with Long COVID symptoms," she said. "And they often have multi-system manifestations."
Beyond brain fog and fatigue, Dr. Hasan reported symptoms including GI problems, rapid heartbeat, dizziness, lightheadedness, aversion to smells, and trouble keeping food down. In the New York and New Jersey area where she practices, she has seen presentations ranging from ringing in the ears and facial paralysis to cardiac enzyme elevation and co-infections such as mono.
Mental Health Affected an Entire Generation
One finding surprised DeBiasi and her colleagues. Children who had COVID-19 (搜索) reported more physical symptoms, more pain, more sleep problems, and greater difficulty with cognitive functioning than children who had not been infected — but rates of anxiety and depression were similar in both groups.
That suggests many of the mental health challenges children experienced over the past several years were not caused by infection alone. "The pandemic has affected all children," DeBiasi said.
Dr. Hasan observed an increasing number of children and teens who developed mental health problems such as anxiety and depression because of the impact that physical symptoms from Long COVID (搜索) have on them. "Teens who had previously aced high school or college courses start struggling with basic math or having trouble staying focused," she said. "They have multiple days of absences and have trouble functioning in a school setting."
No Lasting Heart Damage Found
Parents often worry that COVID-19 (搜索) might leave lasting damage to a child's heart. Researchers specifically looked for evidence of that and found none. Even children who reported symptoms such as chest pain, fatigue, or racing heartbeats generally had normal cardiac function on advanced testing, including an echocardiogram. Additional detailed analysis of EKG and cardiac MRI data is in progress.
For families who continue to worry about long-term heart damage, DeBiasi says these findings should provide reassurance.
Toward Coordinated Care and Prevention
Many of the challenges facing children with Long COVID (搜索) are familiar to clinicians who care for patients with other infection-associated chronic illnesses, including post-treatment Lyme disease syndrome (搜索), myalgic encephalomyelitis/chronic fatigue syndrome (搜索) (ME/CFS), PANS/PANDAS, and other conditions that can persist after an infection has resolved. These patients do not fit neatly into a single specialty; a child may see a cardiologist for chest pain, a neurologist for headaches, a gastroenterologist for abdominal symptoms, and a psychologist for cognitive concerns.
"We hear over and over from these families that people don't believe them," DeBiasi said. She sees an opportunity to recognize infection-associated chronic illness as a clinical category worthy of dedicated expertise, research, and care models.
On prevention, Dr. Hasan emphasized that staying up to date on COVID-19 (搜索) vaccination is one of the best protective measures. "I strongly recommend the vaccine to my patients because it significantly reduces the risk of developing Long COVID (搜索)," she remarked. She also noted that children who had a history of atopy — allergic conditions such as seasonal allergies, asthma, or eczema — were more prone to develop Long COVID, as were children who had prolonged fevers from COVID-19 infection.
Long COVID (搜索) is diagnosed based on a patient's history and findings from a physical examination; there is no testing that can definitively diagnose or rule out the condition. Treatments vary for each person and are individually tailored based on specific symptoms and health needs. Dr. Hasan advises that any child who has COVID-19 (搜索) symptoms for over a month should see their doctor to determine the best treatment plan.
The next challenge, DeBiasi said, is understanding why some children recover quickly while others continue to struggle months or years after infection. Researchers are now using the study's clinical data and biological samples to search for those answers. In the meantime, she hopes the research helps clinicians and families recognize an important reality: while most children recover completely from COVID-19 (搜索), those who do not deserve to be heard, supported, and studied.
"Resources that are applied to adults should be equally applied to kids," DeBiasi said. "There's a specific group of children who need our attention."
