Pennsylvania Reports Four Measles Breakthrough Cases in Fully Vaccinated Residents Amid Largest Outbreak in 30 Years

Pennsylvania health officials reported four breakthrough measles (搜索) cases in fully vaccinated residents, the first such cases in the state's 2026 outbreak.

Four Pennsylvanians who were appropriately vaccinated against measles (搜索) have contracted the virus, state health officials announced Friday, marking the first breakthrough infections among fully vaccinated residents in Pennsylvania's expanding outbreak. The cases occurred in three adults and one child across Centre, Jefferson, Lancaster, and Lycoming Counties, and none required hospitalization.
"Breakthrough cases" — infections in vaccinated individuals — are rare but can occur when a person's immune system "did not respond as well" as most patients' to the vaccine, according to the CDC (搜索). Vaccinated people can also become infected after prolonged exposure to the virus, such as living in a household with multiple unvaccinated people who have measles (搜索). When this happens, vaccinated individuals tend to have milder illness and are less likely to spread measles to others.
The four breakthrough cases represent less than 1% of the total cases reported in the state this year. Before Friday, Pennsylvania had identified no cases in fully vaccinated people.
"When community transmission rises and our overall case numbers increase, we see that a handful of people who were vaccinated will get measles (搜索). Even so, vaccination still provides the best protection," Pennsylvania health secretary Debra Bogen said at a news briefing.
Outbreak Scale and Hospitalizations
Pennsylvania is in the midst of its biggest measles (搜索) outbreak in more than 30 years, with 676 cases reported and 124 people hospitalized as of Friday. The state recorded 52 new cases in the two days preceding the announcement alone. Local and state health officials have noted that many cases are likely going unreported. Lancaster County accounts for 295 of the statewide cases, with a record 49 additional cases reported there in a seven-day period.
The rapidly rising case count, particularly among adults ages 25 to 49, merits attention, said Rémy Pasco, a research associate at the University of Colorado who helped develop a measles (搜索) outbreak simulator based on school vaccination rates. In past national outbreaks, cases have been more common among children too young to have received both MMR doses. Pasco said the pattern could indicate the virus reached a community where both adults and children were largely unvaccinated.
The state has reported two measles (搜索)-associated deaths — defined as deaths where evidence of measles is present, even when the disease may not be assessed by the coroner as the immediate cause of death. The CDC (搜索) had not listed the deaths on its website highlighting data on ongoing measles outbreaks as of Friday's update, and state and federal officials have clashed over whether to tie the deaths to the outbreak.
Lancaster County Coroner Steve Diamantoni investigated one death in an infant with a severe genetic disorder and another in an infant who contracted measles (搜索) before birth and died shortly after. He attributed the latter death to a lacerated spleen rather than measles, though prominent physicians have noted that measles can weaken a person's spleen, putting it at risk of rupture. The second infant had Amish lethal microcephaly and died Aug. 19, according to the coroner.
Vaccination Uptake Rises
Amid the outbreak, Pennsylvania is seeing increased interest in vaccination. Healthcare providers in the Commonwealth typically administer 25,000 doses of the MMR vaccine per month; in August, they gave out 46,000.
"That is 46,000 times Pennsylvanians have chosen to help stop this outbreak by protecting themselves and others," Bogen said.
Vaccine Effectiveness and Breakthrough Risk
People born before 1957, before the first measles (搜索) vaccines became available, are generally considered protected through natural immunity because the disease was once prevalent in children. Between 1968 and 1989, most U.S. children received one dose of the MMR vaccine, which is about 93% effective. In 1989, measles cases among vaccinated schoolchildren prompted health officials to recommend two doses, which together are 97% effective — a change that also led some older children to receive a second shot. Measles rates plummeted during the 1990s, and the disease was declared eradicated in the United States in 2000.
The CDC (搜索) estimates that about three in 100 people who received two recommended MMR doses will contract measles (搜索) if exposed, while about seven in 100 people who received one dose will contract the disease.
The MMR vaccine's 97% effectiveness rate makes it "not perfect, but as good as our vaccines get," said Rochelle Walensky, who served as CDC (搜索) director during President Joe Biden's administration and is currently a professor of medicine at Harvard Medical School. She noted that smaller outbreaks with dozens of cases may not see any breakthrough cases.
"The larger your outbreak, the more likely you're going to see these," Walensky said. "It's just simple math."
Nationwide, 95% of the 3,294 measles (搜索) cases logged by the CDC (搜索) this year have been among unvaccinated people. The agency said 3% of cases occurred in people with one MMR dose and 3% in patients with two doses.
Sarah Long, a professor of pediatrics at Drexel University College of Medicine who previously served on two federal committees to recommend and license vaccines, called cases in vaccinated people "oddities."
"This is a fabulous vaccine and a fabulously transmissible virus," she said, emphasizing that the vast majority of people who get measles (搜索) are unvaccinated.
Milder Disease, Continued Transmission Risk
A Lancaster County pediatrician emphasized that measles (搜索) is less severe for vaccinated individuals. "In the highly unlikely event that someone who is fully vaccinated contracts measles, their illness will be much more mild and less likely to cause serious illness than if they had not received two vaccines," said Pia Fenimore, chief and vice chair of pediatrics at Penn Medicine Lancaster General Health (搜索). She advised that adults and children who are immunocompromised due to cancer or other illnesses should be especially careful, since these treatments and conditions can weaken vaccine efficacy. Individuals uncertain about their vaccination status should contact their healthcare provider, who may suggest checking antibody titers, which "measure the strength of a person's immunity to an infectious agent," Fenimore said.
Breakthrough illnesses can still be contagious, said Paul Offit, a physician and vaccine expert at the Children's Hospital of Philadelphia who heads the hospital's Vaccine Education Center. High levels of community immunity protect everyone living there, Offit said: an unvaccinated person living among mostly vaccinated people is less likely to get measles (搜索) because the virus has less opportunity to spread. But vaccinated people living around many unvaccinated people face higher risk of breakthrough infection due to greater viral circulation.
Long recalled caring for children during a 1991 measles (搜索) outbreak that sickened more than 1,000 people in Philadelphia and killed nine children. "It's a horrible, horrible disease. We are extremely grateful we live in the U.S. where these vaccines are developed and proved to be safe and highly effective," she said.
National Context
Pennsylvania is one of many states that has dealt with measles (搜索), a virus once considered eliminated, this year. As of Sept. 4, South Carolina led the country with 671 cases reported, according to the Johns Hopkins Bloomberg School of Public Health measles tracker. With the rise in cases over the past week, Pennsylvania may now have taken the lead, although the tracker had not yet been updated. Similar breakthrough cases have emerged elsewhere: a vaccinated person in Bucks County contracted a mild case of measles last year after traveling to Texas during that state's major outbreak.
