Persistent Financial Hardship Linked to Poorer Memory, Slower Processing Speed, and Accelerated Brain Aging by Midlife

A landmark study spanning more than seven decades has revealed that chronic financial hardship during working-age adulthood is associated with measurable declines in memory and processing speed by midlife, along with structural brain changes indicative of accelerated aging. Published on July 23, 2026, in the journal Innovation in Aging, the research draws on the Medical Research Council (MRC) 1946 National Survey of Health and Development (NSHD), one of the world's longest-running longitudinal birth cohort studies.
"Most studies on cognitive ageing look at financial hardship at only a single point in time," said Dr. Jacques Wels of University College London. "Our study using several decades of data allows us to see that it is the accumulation of hardship over many years that is linked to the worst cognitive health outcomes, rather than occasional episodes of adversity."
Financial Strain and Midlife Cognitive Performance
The study tracked nearly 2,800 participants, assessing household income at ages 26, 43, and 53—with the lowest 20% classified as low income—and self-reported financial hardships at ages 36, 43, and 53. Both measures were categorized as none, intermittent, or persistent, with persistent exposure defined as hardship recorded at two or more assessment points. Among the analytical sample, 16% experienced persistent low household income and 12% experienced persistent financial hardships.
Cognitive function was measured at ages 53, 63, and 69 using processing speed and verbal memory tasks. The findings showed that greater financial adversity was associated with slower processing speed and poorer verbal memory at age 53, though these associations weakened somewhat after adjusting for confounders. Critically, it was sustained hardship—not brief or isolated episodes—that mattered most, supporting what researchers call the accumulation model of life-course disadvantage, in which repeated stressors compound over time to damage health.
Brain Imaging Reveals Structural Correlates
A subset of approximately 470 participants from the Insight 46 sub-study underwent detailed brain imaging in their early 70s, including magnetic resonance imaging (MRI) and positron emission tomography (PET). Participants with persistently low household income showed larger ventricular volume—a marker often associated with brain tissue loss—at ages 69–71.
While financial adversity alone was not directly associated with changes in brain volume over the roughly two-year follow-up period, several important subgroup interactions emerged. Men with ongoing financial hardships experienced more rapid brain atrophy and ventricular expansion than women. Participants with both a disadvantaged childhood and persistent low household income showed greater hippocampal atrophy and ventricular expansion. Furthermore, APOE-ε4 (搜索) carriers with persistent low income or persistent financial hardships demonstrated faster total brain and hippocampal atrophy and greater ventricular expansion than non-carriers.
"Dementia (搜索) is one of the leading contributors to the global burden of disease, and the rates are estimated to triple to 150 million over the next three decades," Dr. Wels and colleagues noted. "There is a need to better understand how modifiable environmental exposures may be associated with cognitive aging."
Sex, Genetics, and Childhood Circumstances as Modifiers
Sex modified the association between financial adversity and cognition: men with persistent low income performed worse on processing speed at age 53 compared to women. There was also suggestive evidence that men with persistent financial hardships experienced slower processing-speed decline than women, although this interaction did not meet the conventional 0.05 significance threshold.
APOE-ε4 (搜索) carriers who experienced persistent financial hardships had the greatest amyloid burden, though this interaction was likewise suggestive and did not reach conventional significance. No notable interactions were observed for childhood socioeconomic status or genetic risk for cognitive outcomes on the cognitive measures themselves.
Interpretation and Limitations
The authors interpreted the finding that persistent adversity was associated with slower decline in verbal memory as reflecting cognitive losses already present by midlife, rather than a protective effect, because participants with lower initial scores generally showed less subsequent measured decline.
"Our findings suggest that supporting people facing financial hardship and reducing chronic poverty could also help prevent cognitive decline and dementia (搜索) cases in the future," said Professor Praveetha Patalay of University College London.
The researchers acknowledged several limitations, including selective attrition, limited ethnic diversity, a healthier and more socially advantaged imaging sample, categorical income data, and the absence of analyses examining cardiovascular health and other possible pathways. Because participants were born in 1946, the observed sex differences may partly reflect the employment patterns and gender roles of that historical period. The neuroimaging subgroup interactions were exploratory and require replication given the modest sample sizes and multiple comparisons. The observational design does not establish causality, and the study did not test any intervention.
