PLL Therapeutics Advances Gut-Targeting ALS Therapy to Phase II After Positive Safety Results
核心洞察
PLL Therapeutics (搜索) completed a successful phase I/II trial of PL001 in 12 ALS (搜索) patients, demonstrating safety and tolerability with no serious adverse events or treatment discontinuations.
The company's novel polypeptide delivery platform targets gut permeability, addressing the root cause of ALS (搜索) by delivering small-chain fatty acids to strengthen the gut barrier and blood-brain barrier (搜索).
A phase II trial enrolling 140 ALS (搜索) patients across Australia and New Zealand is planned for early 2026, focusing on a region with higher-than-average ALS incidence rates.
PLL Therapeutics (搜索) has reported positive phase I/II safety data for its gut-targeting ALS (搜索) therapy PL001, marking a significant milestone in the company's mission to address the root cause of amyotrophic lateral sclerosis (搜索) through restoration of intestinal barrier function.
The double-blind, placebo-controlled trial evaluated PL001 in 12 ALS (搜索) patients across three different doses administered by subcutaneous injection. The study demonstrated strong safety and tolerability profiles, with no serious adverse events and no treatment-emergent adverse events leading to study discontinuation.
Novel Approach Targets Gut-Brain Connection
PL001 represents a fundamentally different therapeutic approach for ALS (搜索), focusing on the connection between the disease and increased gut permeability. Research has identified links between ALS and "leaky gut (搜索)," a condition where microbes, lipopolysaccharides (LPS), and other toxins can leak into the bloodstream and potentially travel to the brain.
The therapy utilizes PLL Therapeutics (搜索)' proprietary polypeptide delivery platform based on poly-L-lysine (搜索), which serves as a universal carrier that can be conjugated with active compounds. This platform increases the half-life of active compounds in the body and enables them to cross the blood-brain barrier (搜索).
"This is a critical step forward in our mission to restore the intestinal epithelium barrier, thereby treating the root cause of ALS (搜索) – a disease linked to the dysbiosis of the gut," said Jean-Pascal Zambaux, co-founder and CEO of PLL Therapeutics (搜索).
Mechanism Focuses on Barrier Restoration
PL001 works by delivering small-chain fatty acids (SCFAs) to the gut epithelium (搜索) and blood-brain barrier (搜索). Research has demonstrated that SCFAs enhance tight junctions in the gut epithelium, making it more difficult for harmful substances to cross into systemic circulation.
This mechanism represents a departure from currently approved ALS (搜索) medications such as Riluzole and Edaravone, which target neuronal protection and oxidative stress but do not address gut permeability issues.
Phase II Trial Targets High-Incidence Region
The company is advancing to a phase II clinical trial that will enroll 140 ALS (搜索) patients in Australia and New Zealand, scheduled to launch in early 2026. The trial has received approval from the New Zealand Ethics Committee and will be conducted in partnership with local research organizations, including New Zealand Clinical Research (搜索).
New Zealand presents a particularly compelling clinical environment, with ALS (搜索) incidence rates significantly higher than the global average. The country reports approximately 3 cases per 100,000 people compared to the international average of 2 per 100,000 people. Over 400 people live with ALS in New Zealand at any given time, with approximately 120 deaths annually.
PLL Therapeutics (搜索) has established a subsidiary in New Zealand to facilitate patient enrollment, with an immediate goal of recruiting 12 patients at the Pacific Clinical Research Network (搜索). The expansion follows the company's earlier establishment of an Australian subsidiary.
"Our subsidiary in New Zealand will play a pivotal role in our R&D efforts for ALS (搜索) patients in Asia-Pacific," Zambaux noted. "New Zealand's unique position and supportive regulatory environment make it an ideal partner in our mission to bring about a fundamental shift in the standard of care."
Competitive Landscape and Future Development
The gut-targeting approach places PLL Therapeutics (搜索) alongside other companies exploring the microbiome-ALS (搜索) connection. French company MaaT Pharma (搜索) is investigating MaaT033, an experimental oral microbiome therapy that also targets the gut epithelium (搜索). Unlike PLL Therapeutics' approach of delivering molecules to strengthen the gut barrier, MaaT Pharma delivers an ecosystem of bacteria tailored to specific indications. MaaT Pharma released positive safety and tolerability data in late 2024.
Beyond therapeutic development, PLL Therapeutics (搜索) is also advancing a biomarker panel for early-stage ALS (搜索) diagnosis, potentially addressing another critical unmet need in ALS management.
The phase II trial will evaluate PL001's ability to target organs and inflammation while assessing its capacity to restore gut barrier function, providing crucial efficacy data for this novel therapeutic approach in a disease with no current cure.
