The SWOG S1512 trial, conducted by the SWOG Cancer Research Network and funded by the National Cancer Institute, enrolled 28 patients with surgically resectable desmoplastic melanoma (搜索) in cohort A. Participants received three infusions of pembrolizumab, an anti-PD-1 (搜索) immune checkpoint inhibitor, over nine weeks prior to surgery.
Exceptional Response Rates and Long-Term Outcomes
The results showed remarkable efficacy, with 71% of patients achieving a complete pathological response, meaning no detectable cancer remained at the time of surgery. The treatment demonstrated a favorable safety profile, with generally mild side effects reported across the patient population.
Long-term follow-up data revealed sustained clinical benefits. At the three-year mark, 95% of patients had not died from the disease, and 74% remained cancer-free, indicating durable disease control with this neoadjuvant approach.
"We're seeing that desmoplastic melanoma (搜索), which can be challenging to remove surgically, responds extremely well to immunotherapy," said Dr. Antoni Ribas, the study's senior author and professor of medicine at the David Geffen School of Medicine at UCLA. "We found that giving pembrolizumab before surgery is a powerful and safe approach that reduces the need for invasive procedures and improves long-term outcomes."
Addressing a Challenging Clinical Problem
Desmoplastic melanoma (搜索) presents unique treatment challenges due to its aggressive nature and anatomical characteristics. The cancer often develops on sun-exposed areas such as the head and neck, growing deep into tissues and sometimes along nerves, making surgical removal both challenging and potentially disfiguring.
Historically, the standard treatment approach involved wide surgical excision followed by radiation therapy. Advanced disease was considered largely resistant to systemic therapies including chemotherapy and immunotherapy, leaving patients with limited therapeutic options.
Building on Previous Success
The current findings build upon earlier results from cohort B of SWOG S1512, which demonstrated that pembrolizumab could shrink tumors in nearly 90% of patients with advanced, inoperable desmoplastic melanoma (搜索). This success prompted researchers to investigate whether the same therapeutic approach could benefit patients with surgically removable tumors.
The research team collected tissue samples before, during, and after therapy to track tumor response, providing detailed insights into the mechanism of action and treatment effects.
Paradigm Shift in Treatment Approach
Dr. Ribas, who also serves as director of the UCLA Health Jonsson Comprehensive Cancer Center's Tumor Immunology Program and the Parker Institute for Cancer Immunotherapy Center at UCLA, emphasized the transformative nature of these findings.
"Giving pembrolizumab before surgery led to very high rates of tumor clearance, few serious side effects, and excellent three-year survival for patients with this rare and difficult-to-treat cancer," Ribas stated. "Taken together with our earlier findings in patients with advanced disease, these results represent a true shift in how desmoplastic melanoma (搜索) is treated, moving away from repeated surgeries and radiation toward a single therapy that offers durable control, improved survival, and better quality of life."
The study was conducted with Dr. Kari Kendra from The Ohio State University Comprehensive Cancer Center (搜索) as the first author, along with multiple collaborators from UCLA and other institutions. The research received funding from SWOG, the National Institutes of Health, and the National Cancer Institute.
