"I think there's a period of vulnerability where they don't have to be seen as frequently, and the appointments get more spaced out, so they feel like they're not being followed as closely," Olivo explained. "That being said, there are long-term side effects that are still present that need to be monitored over time."
Managing Post-Treatment Symptoms and Long-Term Effects
Survivorship care extends far beyond monitoring for recurrence. Women face a range of persistent symptoms from their treatments, including fatigue, memory difficulties, hormonal changes, and bone health concerns. "Exercise is a huge thing that we recommend. It helps with mobility, and it helps with strength training, which can be helpful for bone health, which can be a consequence of some of the medications that we recommend," she said. "It helps with stress levels. It also helps maintain weight and cardiovascular function, and it also can reduce risk as well."
Memorial Sloan Kettering has established specialized services to address survivorship needs, including a sexual health clinic that helps women manage hormonal symptoms and rebuild intimacy after treatment. The center's approach recognizes that survivorship encompasses both physical recovery and psychological well-being.

"What's great about it is I can encourage patients to complete their therapy if they are gaining a big enough benefit, or vice versa," Olivo noted. "I could avoid over treatment, because if they're really not likely to receive a benefit from the endocrine therapy, we're able to recommend completion of therapy at the five-year mark and spare some of those side effects from the medications."
The BCI addresses a critical clinical challenge, as extended endocrine therapy benefits are often modest and associated with tolerability concerns including hot flashes, muscle and joint pain, osteoporosis, headaches, and mood swings. Evidence supports the predictive benefit of BCI for extended endocrine therapy in patients with 0 to 3 involved lymph nodes, though data remain limited for those with 4 or more positive lymph nodes.
AI Technology Enhances Surveillance Care
Artificial intelligence is beginning to transform breast cancer surveillance through AI-assisted mammography. Recent research published in Radiology demonstrated that AI can reduce radiologist workload by approximately 40% while maintaining a cancer detection rate of 6.6 per 1000 examinations.
"AI is often used to reprioritize radiologists when there's a low likelihood of malignancy," explained study author Sarah Verboom, PhD candidate. "Those are obviously the normal cases where a lot of time is spent because, of course, in screening, the majority of the women are healthy and don't have any abnormalities. If AI can take over some work for those women where there's obviously nothing to see, radiologists can focus their time and attention on the complex cases and find more cancers there."
While more research is needed for AI models to match the sensitivity of double radiologist reading, these findings suggest potential for faster, more accurate mammogram interpretations that could provide survivors with greater peace of mind during routine screenings.

"At every visit, what we do is always tell them, 'Listen, we are here for you.' And I say to all my patients, if at any point, something's just not sitting well with you and you're worried, and it's keeping you up at night, call us and we will talk you through," Olivo said.
This approach emphasizes continuous availability and open communication, helping survivors feel supported even as their care transitions from intensive treatment to long-term monitoring.
Moving Toward Precision Survivorship Care

"[The BCI] takes the guesswork out, and it really gives them the information they need to make an informed decision," Olivo explained. "It could also help avoid over- and undertreatment again because it's not always so black and white, as the lower-risk-profile patients automatically are not going to benefit from the full 10 years."
However, the promise of precision survivorship care raises important questions about equity and access. For these advances to achieve their full impact, they must be available beyond major academic centers to reach community clinics and rural hospitals where many survivors receive care.
