Psilocybin Shows Unexpected Cognitive Improvements in Advanced Alzheimer's Case Report

A woman in her 80s with advanced Alzheimer's disease (搜索) experienced striking, temporary improvements in speech, continence, and mobility after receiving psilocybin-containing mushrooms, according to a case report by neuroscientists in Brazil. The findings, while limited to a single patient, have prompted researchers worldwide to ask whether the psychedelic compound could unlock residual brain function in people with severe neurodegeneration.
The patient, a Japanese-American woman, had experienced progressive cognitive decline for a decade. For the previous five years, she had largely communicated using single words and relied heavily on others for everyday care. She also had difficulty walking and dressing herself and experienced chronic urinary incontinence.
Under clinical supervision in Brazil, with written informed consent from her legal guardian, the woman received 5 grams of psilocybin-containing mushrooms. The exact amount of psilocybin is unclear because mushroom potency varies. During the experience, she sweated heavily and entered a prolonged, deep sleep-like state.
Roughly 19 hours after the oral dose, the woman suddenly began to talk to herself and continued doing so for several hours. Over the following days and weeks, caregivers reported that she seemed more alert, recognized family members, walked more independently, began dressing herself, and regained urinary continence.
"The persistence of urinary continence after more than 5 years of chronic incontinence is particularly notable, given that continence depends on integrated interoceptive awareness, executive inhibition, and fronto-insular network function," wrote the researchers, including neurologists Mariana Cerveira and Joe Xavier Simonet.
One month later, the patient received a second supervised session involving 3 grams of mushrooms. During this session, she did not fall asleep but remained verbally expressive throughout, describing emotional scenes such as surfing with her son on a peaceful island. "Facial expressivity, emotional reciprocity, spontaneous humor, and gait agility appeared markedly improved," the case report noted. During this second session, the woman said, unprompted, "It is pleasant to come here."
Not a Reversal of Pathology
The authors, led by neuroscientist Marcos Lago from the University of São Paulo (搜索), explicitly cautioned against overinterpretation. "The findings should not be interpreted as a reversal of Alzheimer's pathology," they wrote. "Rather, they raise the possibility that latent functional capacities may persist in advanced neurodegeneration and become temporarily accessible under specific neuromodulatory conditions."
The report carries significant limitations. It involved one person rather than a controlled clinical trial. The diagnosis was based on clinical history rather than confirmed using biomarkers such as brain scans or spinal fluid analysis. There was no comparison group and no standardized testing of memory and thinking before and after treatment. Observations were largely based on reports from caregivers and family members. Researchers did not monitor the woman's sleep state or brain activity during her experience.
"The present report should be understood primarily as a detailed observational description intended to generate hypotheses for future controlled investigation," the authors wrote. "Systematic investigation is warranted."
Mechanisms Under Investigation
Alzheimer's disease (搜索) involves abnormal proteins, inflammation, damage to connections between brain cells, and ultimately neuronal death. There is no evidence that psilocybin reversed these underlying disease processes. The authors hypothesize that psilocybin may temporarily have altered communication between surviving brain networks, making some abilities more accessible for a limited period. Because the report did not include brain scans, this remains untested.
Psilocybin acts mainly through the serotonin receptor 5-HT2A (搜索). Studies in animals suggest that psilocybin can encourage the formation of dendritic spines—tiny protrusions on nerve cells that help them communicate. Psychedelics may also affect signaling pathways involving brain-derived neurotrophic factor (BDNF), a protein involved in maintaining nerve-cell connections. Brain-imaging studies suggest that psilocybin temporarily changes communication between large-scale brain networks, with some networks becoming less rigidly separated.
Separate research at the University of California, Berkeley, is examining how psilocybin affects cognitively healthy adults aged 60 to 85. The study is not testing a dementia treatment but will involve synthetic psilocybin, brain scans, and tests of memory and thinking. Additionally, one pilot study is evaluating whether psilocybin, provided in a supervised environment, can reduce depression and improve quality of life in people with mild cognitive impairment or early-stage Alzheimer's disease (搜索).
Safety Considerations
The case report also highlights important safety concerns. Psilocybin is not risk-free. Psychedelic experiences can be frightening and disorienting, particularly for vulnerable people. Older adults may face increased risks of falls, heart and circulation problems, and interactions with medications. The woman in this case experienced heavy sweating, suspected high body temperature, and a prolonged sleep-like state. The absence of lasting complications does not establish that the approach is safe.
The selected mushroom dose was relatively high compared with dosing approaches commonly used in modern clinical trials. The authors explained that the dose "was chosen based on prior experiential observations regarding depth and duration of psychedelic-induced neurobehavioral effects." Future randomized clinical trials are needed to determine whether psilocybin can produce meaningful benefits and at what dose it is safest and most effective.
The case has drawn comparisons with neurologist Oliver Sacks's 1973 book Awakenings, which described patients who unexpectedly regained lost abilities after treatment with the Parkinson's drug L-dopa. While the diseases and drugs are entirely different, both raise questions about how much function may remain hidden within a damaged brain. Answering those questions for psilocybin and Alzheimer's disease (搜索) will require controlled research.
