Real-World PROSPER Data Show Mogamulizumab Provides Rapid, Sustained Symptom Relief in Mycosis Fungoides and Sézary Syndrome

Kyowa Kirin announced positive results from PROSPER, a real-world observational study evaluating mogamulizumab (POTELIGEO) in adults with relapsed or refractory mycosis fungoides (搜索) (MF) or Sézary syndrome (搜索) (SS), at the 6th World Congress of Cutaneous Lymphomas in Montreal, Canada. The findings demonstrate that mogamulizumab delivers clinically meaningful improvements in skin symptoms and health-related quality of life (HRQoL) as early as four weeks after treatment initiation, with benefits sustained throughout the 48-week study period.

Study Design and Patient Population
PROSPER (ClinicalTrials.gov ID NCT05455931) enrolled 73 patients with relapsed or refractory MF (n=41) or SS (n=32) across 19 sites in six countries spanning North America, Europe, and the Middle East. The study was designed with direct input from patients and caregivers to ensure patient-relevant outcomes were captured. Patients were followed for up to 50 weeks from enrollment.
Patient-reported outcomes were collected at baseline and throughout the study using three instruments: a CTCL-specific symptom diary assessing severity of skin itch, pain, redness, flaking, sleep problems, and body temperature regulation difficulties; the MF/SS-CTCL-QoL questionnaire measuring CTCL's impact on daily life; and the Brief Fatigue Inventory (BFI) evaluating fatigue severity and its functional impact.
Rapid and Sustained Symptom Improvement
Mean symptom scores improved from baseline to week 48 across all key skin symptoms. Itch scores decreased by −2.5 (±3.4), flaking by −3.1 (±3.5), redness by −3.1 (±3.5), and pain by −1.7 (±4.4). Clinically meaningful improvements—defined as exceeding the minimum important difference (MID)—in skin itch, flaking, and redness were observed as early as week 4, with pain improvements reaching the MID threshold by week 12. These effects were sustained through week 48.
Beyond skin symptoms, the study captured meaningful functional improvements. By week 48, 30% of patients reported at least a 2-point improvement in sleep, and 37% reported better body temperature regulation. Patients also demonstrated significant, clinically meaningful improvements in disease-specific HRQoL scores (MF/SS-CTCL-QoL) beginning at week 12, with additional gains accumulating through week 48.
Fatigue outcomes differed by subtype. While fatigue scores changed little among patients with MF, patients with SS showed a significant improvement in total BFI scores, reaching the MID threshold at week 48.

Clinical Context
Mycosis fungoides (搜索) and Sézary syndrome (搜索) are two subtypes of cutaneous T-cell lymphoma (搜索), a rare form of non-Hodgkin lymphoma that primarily affects the skin, presenting as patches, plaques, tumors, or erythroderma, and may be associated with severe itching. The disease can spread to lymph nodes, blood, and other organs in some patients. Mogamulizumab is indicated in the U.S. for the treatment of adult patients with relapsed or refractory MF or SS after at least one prior systemic therapy.
