Baroness Heather Hallett, Chair of the COVID-19 Inquiry, delivered this assessment while presenting the findings of a 274-page report examining the UK's pandemic response.
The trial's most consequential finding — that dexamethasone, an inexpensive and widely available steroid, dramatically reduced mortality in hospitalized COVID-19 (搜索) patients — reshaped clinical practice globally within hours of the results being announced. By March 2021, the drug had saved an estimated 22,000 lives in the UK and approximately 1 million across the globe.
A Trial Built at Unprecedented Speed
The RECOVERY trial was established in just nine days after being designated an urgent public health research study. Within two months, 10,000 patients had been recruited. The study focused on repurposing existing medications — drugs already approved for other conditions — to determine whether they could be effective against the novel coronavirus.
Within 100 days, the trial delivered its first results. Two drugs being widely used at the time — hydroxychloroquine and lopinavir-ritonavir (搜索) — were shown not to improve survival. In contrast, dexamethasone was found to be life-saving. Hospitals began dispensing the steroid to COVID-19 (搜索) patients within hours of the results confirming its effectiveness.
The Clinical Reality on the Ground
Dr. Maheshi Ramasamy, a clinician who treated patients during the early waves, described the desperate circumstances that made the trial so urgently necessary. "It was horrendous. It was like being in a war," she recalled. "We had no idea how to manage this clinically, and that felt really disempowering — these desperately unwell people and nothing really that you could offer them."
The decision to test dexamethasone, a potent immunosuppressant steroid, was not without controversy at the time. "It seemed quite a brave decision to actually try an immunosuppressant to dampen your immune response against a virus," Ramasamy noted. "Actually, very quickly, it became apparent because of the RECOVERY trial that dexamethasone was actually phenomenally good at saving lives."
Scale and Legacy
RECOVERY went on to become the largest clinical trial for COVID-19 (搜索) treatments in the world, recruiting over 48,000 patients across 177 NHS trusts as well as international sites. The trial evaluated a total of 16 drugs and identified four effective treatments.
The late Professor Sir Nicholas White, an expert witness to the Inquiry, described the dexamethasone finding as "the single most important therapeutics research result of the pandemic" and a "remarkable achievement."
Sir Martin Landray, Professor of Medicine and Epidemiology at Oxford Population Health and Joint Chief Investigator for the RECOVERY trial, emphasized the collaborative foundation of the study's success: "A collaborative approach was critical to our success — working with research networks across the UK, integrating the trial into routine care, and working at pace with regulators, enabled us to deliver results in the most exceptional circumstances."
Preparing for Future Pandemics
The Inquiry report highlighted that the UK research response drew upon pre-existing trial infrastructure established in previous health emergencies. The RECOVERY trial was informed by one of eight "hibernated" studies commissioned by the National Institute of Health and Care Research (搜索) following a review of the 2009 pandemic influenza outbreak.
Baroness Hallett's report calls for continued investment in biomedical research and development, with a clear message: "Vaccines alone are not enough. Therapeutics are an essential tool in the response armoury."
Sir Peter Horby, Moh Family Foundation Professor of Emerging Infectious Diseases at the Pandemic Sciences Institute, University of Oxford, and Joint Chief Investigator, reinforced this imperative: "The report highlights the need for investment to support research and development capabilities, to maintain the close links between government, academia and industry, and to be able to pivot easily to combat new pathogens. RECOVERY demonstrated the power of a flexible, streamlined approach to produce reliable results quickly and save lives."
