In a recent conversation with Pharmaceutical Executive, Hilberdink — who previously spent 26 years at Novo Nordisk and has worked in obesity since the earliest days of modern pharmacologic treatment — laid out a vision for the next phase of obesity innovation that moves decisively beyond the bathroom scale.
"Obesity (搜索) is evolving kind of similar to the way Type Two Diabetes evolved," Hilberdink said. "Once upon a time we blamed the patient for not taking care of themselves and giving themselves type two diabetes, and then we understand that it's a much more complex metabolic disease. And I think we're finally at that tipping point now."
From Pounds Lost to Metabolic Health Gained
Hilberdink contends that the industry's singular focus on weight reduction as the primary endpoint has outlived its usefulness. Instead, he advocates for a more sophisticated assessment of treatment success — one that interrogates the quality of weight loss and its impact on key organ systems.
"The next evolution really should be about evolving beyond just looking at pounds on the scale as an indicator of success, and really looking at overall metabolic health," Hilberdink explained. He highlighted three critical parameters: reducing visceral fat — the so-called "bad fat" that accumulates around the abdomen and surrounds vital organs — ensuring liver health, and preserving lean muscle mass during treatment.
The liver's role is particularly consequential. Hilberdink noted that approximately 75% of patients living with obesity (搜索) already have metabolic dysfunction-associated steatotic liver disease (MASLD (搜索)), a condition he described as "highly problematic, because if you don't have a healthy liver, it's going to be very difficult to have overall strong metabolic health."
This reframing carries significant implications for both individual patients and the healthcare system at large. "We know that obesity (搜索) remains a significant burden on the healthcare system," Hilberdink said, pointing to the broad constellation of comorbidities extending beyond diabetes and cardiovascular disease. "If we can really get to the root of the problem and help patients with this complex metabolic disease, we can hopefully avoid a lot of those comorbidities and a lot of the massive downstream cost on society."
Survodutide: A Novel Mechanism of Action
Boehringer Ingelheim's deliberate strategy in the obesity (搜索) space centers on survodutide, a novel glucagon GLP-1 (搜索) dual agonist that the company is preparing to bring to market. Hilberdink described the molecule as having "a unique mechanism of action" with the potential to reduce fat in the liver, reduce visceral fat, and help preserve lean muscle mass — "potentially in a different way than what current therapies are doing."
The therapy is administered as a once-weekly injectable, a dosing schedule that Hilberdink characterized as a significant stride forward from the once-daily injectables that preceded it.
Beyond survodutide, Boehringer Ingelheim is advancing a broader pipeline. "We do have a triple agonist that's currently in development, which would be a first in class, and we're looking at other things in our pipeline," Hilberdink revealed, underscoring the company's long-term commitment to the metabolic health space.
The Access Barrier and the Adherence Challenge
On the question of whether oral or injectable GLP-1 (搜索) therapies represent the future of obesity (搜索) care, Hilberdink offered a pragmatic perspective that diverges from much of the current industry debate.
"I do not believe that the injection barrier is the primary barrier," he stated. "I think that an oral is a good option for patients, but I think again the primary barrier is access to treatment." He pointed to structural obstacles including limited insurance coverage, employer carve-outs, and high out-of-pocket costs as the real determinants of whether patients can benefit from these therapies.
Equally concerning is the issue of treatment persistence. Hilberdink noted that "the average patient is only staying on treatment for six months, and then maybe cycling to another treatment, and going through this roller coaster of gaining weight, losing weight, gaining weight." Breaking this cycle, he argued, requires defining treatment goals that go beyond pounds on the scale and providing comprehensive support around pharmacological intervention.
"We don't want anyone to think that a pharmacological intervention alone is what's going to help a patient get on this journey of being in healthy weight and overall metabolic health," Hilberdink emphasized.
Building on a Heritage in Metabolic Disease
Boehringer Ingelheim's entry into the obesity (搜索) arena is not a departure but an extension of the company's established presence in metabolic health, anchored by its well-known drug in type 2 diabetes (搜索) and cardiovascular chronic kidney disease. Hilberdink framed the obesity strategy as leveraging "some of the knowledge that we have in the disease state to maybe provide something a little broader than what we currently see in the marketplace."
