Risk-Reducing Surgeries Significantly Improve Survival in BRCA-Positive Breast Cancer Patients

A global study presented at the San Antonio Breast Cancer (搜索) Symposium has demonstrated that young breast cancer patients with BRCA mutations can substantially reduce their risk of cancer recurrence and death by undergoing preventive surgical interventions.
The research, led by Dr. Matteo Lambertini from the University of Genova-IRCCS Policlinico San Martino Hospital (搜索) in Italy, analyzed data from nearly 5,300 breast cancer (搜索) patients with BRCA mutations treated at 109 hospitals across five continents. All participants were diagnosed with breast cancer at age 40 or younger between 2000 and 2020.
"This global study provides the first evidence that risk-reducing surgeries improve survival outcomes among young BRCA-mutation carriers with a prior history of early-onset breast cancer (搜索)," said Dr. Lambertini.
Significant Survival Benefits from Preventive Surgeries
The findings revealed that breast cancer (搜索) patients aged 40 or younger with BRCA mutations had a 35% lower risk of death and 42% lower risk of cancer recurrence following risk-reducing mastectomy (breast removal). Similarly, patients who underwent oophorectomy (ovary removal) experienced a 42% lower risk of death and 32% lower risk of recurrence.
Among the study participants, nearly 3,900 underwent at least one risk-reducing surgery, with more than 1,800 opting for both breast and ovary removal. The research team observed that women with BRCA1 (搜索) mutations benefited more significantly than those with BRCA2 (搜索) mutations, with their risk of death reduced by 56% compared to 15%.
Ovary Removal Shows Substantial Benefits Without Expected Side Effects
A subsequent study published in The Lancet Oncology (搜索) on May 7, 2025, provided further evidence supporting the benefits of bilateral salpingo-oophorectomy (BSO) – the removal of ovaries and fallopian tubes – in BRCA carriers with a history of breast cancer (搜索).
The research, conducted by a team from the University of Cambridge, identified more than 3,400 British cancer survivors carrying cancer-causing variants of BRCA1 (搜索) or BRCA2 (搜索) genes. Of these women, approximately 850 BRCA1 carriers and 1,000 BRCA2 carriers had undergone BSO.
Over an average follow-up period of nearly six years, women who underwent BSO were about half as likely to die from cancer or any other cause. Interestingly, BRCA2 (搜索) carriers showed greater benefit, with a 56% reduction in death risk compared to a 38% reduction for BRCA1 (搜索) carriers. The surgery also made women 40% less likely to develop a second cancer.
"Our findings will be crucial for counseling women with cancer linked to one of the BRCA1 (搜索) and BRCA2 (搜索) variants, allowing them to make informed decisions about whether or not to opt for this operation," said senior researcher Antonis Antoniou of the University of Cambridge's Department of Public Health and Primary Care.
Addressing Concerns About Long-term Health Impacts
A significant finding from the Cambridge study was the absence of increased long-term health problems associated with ovary removal. Previous concerns had centered around potential adverse effects such as heart disease, stroke, and depression resulting from early menopause triggered by the loss of estrogen production.
"We know that removing the ovaries and fallopian tubes dramatically reduces the risk of ovarian cancer (搜索), but there's been a question mark over the potential unintended consequences that might arise from the sudden onset of menopause that this causes," explained lead researcher Hend Hassan, a doctoral student at the University of Cambridge Center for Cancer Genetic Epidemiology.
"Reassuringly, our research has shown that for women with a personal history of breast cancer (搜索), this procedure brings clear benefits in terms of survival and a lower risk of other cancers without the adverse side effects such as heart conditions or depression," Hassan added.
Context of BRCA Mutation Risks
Women with BRCA1 (搜索) and BRCA2 (搜索) gene mutations face substantially elevated cancer risks. According to the National Cancer Institute, more than 60% of women who inherit a BRCA mutation will develop breast cancer (搜索) in their lifetimes. Additionally, up to 58% of women with a BRCA1 mutation and 29% of women with a BRCA2 mutation will develop ovarian cancer (搜索).
These high-risk statistics have led to the strategy of prophylactic removal of at-risk tissue – breasts or ovaries – to reduce cancer risk. While this approach has been shown to help women with BRCA mutations who haven't had cancer, its effectiveness in young women who have already developed breast cancer (搜索) had not been thoroughly evaluated until these recent studies.
Balancing Benefits Against Quality of Life Considerations
Dr. Lambertini emphasized the importance of considering the negative effects of these surgeries, particularly ovary removal, which leads to infertility and early menopause.
"Considering the unique traits and needs of this younger population, and their high risk for secondary malignancies, it is critical to understand how risk-reducing surgeries affect patient outcomes, so that the risks and benefits of these procedures can be carefully weighed," he noted.
The Cambridge researchers expressed concern about disparities in surgical uptake among different groups of women. "Given the clear benefits that this procedure provides for at-risk women, it's concerning that some groups of women are less likely to undergo it," Hassan said. "We need to understand why this is and encourage uptake among these women."
