Scholar Rock Advances Apitegromab Toward September 30 PDUFA Date as First Muscle-Targeted Therapy for SMA

Scholar Rock Holding Corporation (搜索) is entering the final stages of the U.S. regulatory process for apitegromab, its investigational muscle-targeted monoclonal antibody designed to inhibit myostatin (搜索) activation in skeletal muscle, with a Prescription Drug User Fee Act (PDUFA) target action date of September 30, 2026, for the treatment of spinal muscular atrophy (搜索) (SMA).
"We are on the threshold of bringing the world's first muscle-targeted therapy to children and adults living with SMA," said David Hallal, Board Chair and Chief Executive Officer, during the company's second quarter 2026 earnings call held on August 6, 2026.
Regulatory Path and Manufacturing Redundancy
The Biologics License Application (BLA) for apitegromab was resubmitted on March 30 with two independent fill/finish facilities — Catalent Indiana and a second fill/finish facility — providing what management described as a "belt and suspenders" approach to approval. The sole approvability issue noted in the complete response letter (CRL) received on the prior priority review PDUFA date last September related to observations identified at a routine general site inspection of the Catalent Indiana fill/finish facility, which is owned and operated by Novo Nordisk.
The FDA inspection classification for Catalent Indiana following the April 2026 general site inspection remains pending. Akshay Vaishnaw, President of Research and Development, noted, "We had anticipated the classification in late July within 90 days following inspection completion based on the agency's guidelines," while acknowledging that the status drift remains an ongoing uncertainty.
The second fill/finish facility now holds more vials available for commercial launch than the Catalent Indiana facility, providing a redundant supply chain. According to management, this facility has had recent successful site inspections by both the FDA and the EMA, and none of the approvals from this facility in the last 12 months required a pre-approval inspection or pre-license inspection.
Clinical Evidence and Unmet Need
The pivotal SAPPHIRE trial demonstrated statistical significance on the Hammersmith motor function scale, with a p-value of 0.019. In the study, patients were randomized while receiving ongoing SMN-targeted therapy to receive either placebo or apitegromab.
The unmet medical need in SMA remains substantial. Despite approximately 78% of children and adults living with SMA in the U.S. receiving an SMN-targeted therapy, 95% of patients continue to experience persistent and progressive muscle atrophy that limits both function and independence. Data shared by Cure SMA indicate that an estimated one-third of people living with SMA in the U.S. have received two or more SMN-targeted treatments, either sequentially or in combination.
Globally, an estimated 35,000 patients have received a survival motor neuron (搜索) (SMN)-targeted therapy and could potentially benefit from muscle-targeted treatments.
Pipeline Expansion
Scholar Rock is advancing apitegromab into additional neuromuscular indications. The company announced the initiation of FORGE, a Phase II randomized, double-blind, placebo-controlled trial evaluating apitegromab in facioscapulohumeral muscular dystrophy (搜索) (FSHD), a rare inherited muscular dystrophy with no approved therapies. The trial will enroll 60 patients, with dosing of 10 mg/kg intravenously or placebo every four weeks for a total of 52 weeks.
Management prioritized FSHD as the next indication because approximately 20% of patients eventually become wheelchair dependent. The trial focuses on patients with a Ricci score of 1.5 to 3, targeting a mild-to-moderate patient population.
The Phase II OPAL trial is evaluating apitegromab in infants and toddlers with SMA under the age of 2, including participants treated with an SMN1-targeted gene therapy or receiving ongoing SMN2-targeted treatment. "The rate at which the [OPAL] study is enrolling underscores the significant unmet need and the potential for apitegromab in the youngest of [SMA] patients," Vaishnaw stated.
Additionally, SRK-439, a high-potency anti-myostatin (搜索) antibody, is progressing in a Phase I healthy volunteer study, with top-line data anticipated in late 2026. A high-concentration subcutaneous formulation of apitegromab demonstrated favorable bioavailability and a pharmacodynamic profile comparable to intravenous administration in Phase I data shared in January, with regulatory engagement planned for later this year.
Commercial Readiness
The U.S. commercial organization is targeting approximately 140 SMA treatment centers and 2,600 prescribing physicians for the upcoming launch. Market access teams have engaged national and key regional payers, along with Medicare and Medicaid, to achieve broad reimbursement for eligible patients.
Chief Operating Officer Robert Keith Woods cautioned that the first six months of launch may experience delays, noting that "the average time from prescription to infusion is typically greater than 60 days without an established J-code." The company expects prior authorization requirements for apitegromab, consistent with the nearly 100% prior authorization rate observed across the three available SMN-targeted therapies.
In Europe, the company is executing a commercial playbook in Germany to establish an initial footprint as part of a planned 50-country operating platform. The apitegromab Marketing Authorisation Application (MAA) currently includes only the Catalent Indiana fill/finish facility, with the EMA awaiting the FDA inspection classification for that site.
Financial Position
Scholar Rock reported a net loss of $109.9 million for the second quarter of 2026, relatively flat compared to the $110.0 million loss in the prior year period. Net loss per share was $0.84, compared to $0.98 in the second quarter of 2025. The company ended the quarter with $492.1 million in cash, cash equivalents, and marketable securities, including $62.8 million in net proceeds from an at-the-market program.
Operating expenses were $108.9 million, including $58.2 million in research and development expense and $50.7 million in general and administrative expense. Management intends to access $150 million in potential debt facility drawdowns upon FDA approval of apitegromab and plans to monetize a priority review voucher to further strengthen the balance sheet.
