The study failed to meet its primary endpoint, with treatment showing no meaningful improvement in remission rates compared to placebo. Two different drug regimens led to remission rates of 10.3% and 10.6%, respectively, versus 11.6% for placebo. The drug also failed to eliminate patients' symptoms any better than placebo and was associated with more side effects.
Safety Concerns Emerge
Safety data revealed concerning trends, with four participants who received Seres' drug experiencing serious adverse events compared to only one who received placebo. These serious events included congestive heart failure (搜索) and the formation of abnormal polyps on the colon. While the majority of treatment-related side effects were mild to moderate, the higher incidence of serious events raised additional safety questions.
As a result of the negative findings, Seres closed the open label and maintenance portions of the trial. Company CEO Eric Schaff expressed surprise and disappointment with the preliminary results, noting how much they diverged from Phase 1 data.
Market Impact Spreads Across Sector
The failure has sent shockwaves through the microbiome therapeutics sector. Seres shares fell by more than half following the announcement, erasing gains the biotech had made since last year. The selloff extended beyond Seres, with rival microbiome therapy companies Finch Therapeutics (搜索), Evelo Biosciences (搜索), and Kaleido Biosciences (搜索) experiencing stock price declines of roughly 20%.
"People don't believe in the translation of the science outside of [C. difficile infections (搜索)] now," said Chris Howerton, an equity research analyst at Jefferies who covers Seres Therapeutics. "I'd expect high scrutiny on microbiome companies now and fundraising to become more difficult."
Company Plans Path Forward
Despite the setback, Seres isn't abandoning ulcerative colitis (搜索) treatment. The company is exploring a path forward for SER-287 and has more data expected by the second half of the year, which may help determine next steps. Schaff indicated the company would take a similar analytical approach that previously helped them overcome a 2016 failure in C. difficile infections (搜索), where they identified issues in patient screening and designed a new trial that yielded positive results in 2020.
"While we don't know what drove the differences between the [Phase] 1 and [Phase] 2 today, we do know that taking a similar process is the right next step for us and that's what we're going to do," said Schaff.
Next-Generation Approach in Development
Seres can fall back on SER-301, a second treatment in early testing that represents a different approach. Unlike SER-287, which is derived from donors, SER-301 is a consortium of bacteria that Seres hand-picked, optimized and cultivated. Chief scientific officer Matthew Henn suggested this more targeted treatment approach may be necessary for diseases like ulcerative colitis (搜索). An early-stage study for SER-301 is currently underway.
Broader Field Context
The failure comes at a time when the microbiome therapeutics field had been gaining momentum. Prior to this setback, Seres had declared success in a Phase 3 study for recurring bacterial infections last August. Combined with positive findings from Finch Therapeutics (搜索) and Rebiotix (搜索), these results had signaled growing promise for microbiome drugs designed to change interactions between the body and its resident microbes.
The success had attracted significant investment, with partner Nestle Health Science (搜索) investing $500 million more in Seres' experimental medicine. However, the ulcerative colitis (搜索) failure represents an important test case for the field's broader applicability beyond bacterial infections.
Other companies continue development in inflammatory bowel disease (搜索), with Vedanta Biosciences (搜索) planning to start a Phase 2 trial in ulcerative colitis (搜索) later this year, while Finch's program remains in preclinical testing.
